    Position Overview:

    The Client Care Liaison is responsible for overseeing and managing the day-to-day operations of our home care services in compliance with Oregon state laws and regulations. This role ensures that our clients receive high-quality care and that our caregivers are properly trained and supported. The Client Care Liaison reports to the Director of Operations and works closely with the Care Coordinator and other team members to deliver exceptional service to our clients.

    Key Responsibilities:

    Compliance: Ensure compliance with all relevant Oregon state laws and regulations.

    Supervision: Provide supervision and support to Caregivers, including conducting regular supervisory visits and ensuring compliance with care plans. Take part in caregiver evaluations as well and be able to address concerns in a timely manner.

    Training: Coordinate and conduct training for Caregivers during weekly orientation and ongoing as the Caregivers need support.

    Client Care: Monitor client care plans and collaborate with the Care Coordinator to ensure that clients' needs are being met. Client care plans are updated within the first 30 days of service and every 90 days. Meet with potential new clients to discuss services and complete service agreements.

    Quality Assurance: Implement quality assurance measures to ensure the delivery of high-quality care services.

    Documentation: Maintain accurate and up-to-date documentation, including client records, caregiver schedules, and incident reports.

    Communication: Communicate regularly with clients, caregivers, and the management team to address any issues or concerns.

    Risk Management: Identify and mitigate risks related to client care and caregiver performance.

    Reporting: Prepare and submit reports as required by state regulations and company policies.

    Professional Development:Stay informed about developments in the home care field and participate in professional development opportunities; such as marketing events, job fairs, etc.

    Qualifications:

    Experience: Minimum of 2 years of experience in home care or a related field, preferred with at least 1 year in a supervisory role.

    Knowledge: Thorough understanding of Oregon state laws and regulations related to home care.

    Skills: Strong leadership, communication, and problem-solving skills.

    Other Requirements: Valid driver's license and access to a reliable vehicle.

    Spanish Preferred, but not required.

    Physical Requirements:

    • Ability to travel to client homes and caregiver locations within the service area of the office.

    • Ability to lift and move up to 50 pounds.

    Work Environment:

    • This position involves regular travel within the community

    • In Person: Monday thru Friday work schedule with 8am-5pm; full time

    • Able to rotate being on call during the week and weekends

    Salary and Benefits:

    • Competitive salary based on experience: $24.00 - $27.00 per hour
