    Role Overview

    The Client Engagement and Community Liaison Specialist will serve as a vital link between potential clients, their families, and our team. The role involves assisting clients in applying for Medicaid home care benefits, coordinating referrals and the intake process, and fostering partnerships with community organizations to increase awareness and referrals.

    Key Responsibilities

    Community Liaison Work :

    • Build and maintain relationships with community organizations, hospitals, and other referral sources.
    • Represent Helping Hand Nurse, LLC at community events, networking opportunities, and outreach initiatives.
    • Educate community partners about our services to drive referrals and expand our client base.

    Client Engagement and Marketing :

    • Handle inbound and outbound inquiries to qualify leads and convert them into clients.
    • Maintain detailed records of client interactions and progress in the CRM system.
    • Provide exceptional customer service to address client needs and concerns.

    Medicaid Application Assistance :

    • Guide clients through the process of applying for Medicaid home care benefits.
    • Offer support by explaining requirements, collecting documentation, and following up on progress.
    • Act as a liaison with Medicaid offices to resolve application challenges.

    Referral and Intake Coordination :

    • Coordinate with internal teams to ensure smooth client onboarding and timely case starts.
    • Schedule assessments and follow up on all required steps for new cases.
    • Ensure compliance with timelines and provide clients with regular updates.

    Reporting and Collaboration :

    • Track and report on key metrics, including referrals, conversion rates, and community outreach activities.
    • Identify opportunities for growth through client feedback and community trends.

    Qualifications

    • Proven experience in sales, customer service, or community engagement (homecare or healthcare experience preferred).
    • Strong relationship-building and communication skills.
    • Knowledge of Medicaid home care benefit processes is highly desirable.
    • Ability to multitask and prioritize in a fast-paced environment.
    • Familiarity with CRM systems and Microsoft Office Suite.
