    We are looking for a Clinical Care Coordinator to join our team to provide their skill set and knowledge of medication management. Overseeing the care that field staff are giving through our MAR system and equipping the team with resources of how to best serve our members.

    Job duties include, but are not limited too:

    Educational Requirements: Certified Medication Aide (CMA) or Licensed Practical Nurse (LPN) in good standing in the State of Iowa. CPR certification required. Experience: At least one year of experience caring for individuals with intellectual and developmental disabilities (IDD) or mental illness (MI) is preferred. Administrative and supervisory experience is preferred. Skills: Strong knowledge of general nursing practices, including medications, biological, physical, social, and medical sciences.Excellent communication skills, both verbal and written. Ability to manage and supervise a team. Organizational proficiency with the ability to maintain accurate records, including client charts, medication records, and incident reports.Experience in providing hands-on care and the ability to assess and manage client needs effectively. Strong problem-solving skills and ability to work under pressure. Ability to adhere to infection control techniques and confidentiality protocols.

    Job Duties include but are not limited to:

    Communication with guardian and case manager regarding the condition and progress of the tenant, including in-person discussion and answering of telephone inquiries.

    Reviewing physicians’ orders, assisting clients with re-ordering medications, maintenance of the clients’ charts, and transcription of physicians’ orders to MARs.

    Requisition of the necessary supplies, equipment, linens, emergency drugs, and other materials to meet the program needs. This also requires that the Clinical Care Manager be responsible for making sure the equipment is in good working order.

    Maintains records, including charting duties and maintenance of narcotic records.

    Responsible for work assignment of individuals with authority to make changes and revise duties in order to meet client needs. Responsible to ensure that staff members complete assigned duties.

    Responsible for authorization overtime, disciplining employees and recommendation of termination. Assisting Administrator with employee evaluations for all employees in all departments. Where necessary, the Clinical Care Coordinator is to provide counseling to employees on performance improvement.

    Assist with orientation and training programs for new employees.

    Assist the Administrator with implementation of policies and procedures for the program.

    Evaluate emotional and physical needs of tenants.

    Ability to reasonably conform to all rules and policies of the program. Know all policies and procedures.

    Provide privacy of tenants at all times.

    Follow up on all PRN’s.

    Practice infection control techniques; good hand washing, gloves, and disposal of infectious wastes and needles.

    Notify funeral homes when clients expire.

    Assist with transfers, repositioning and ambulating as determined in the clients service plan.

    Check for expiration dates on all medications.

    Oversee Activity Calendar.

    Call therapist with new orders. Follow up with therapist recommendations as determined in the clients service plan.

    Communicate with pharmacy regarding medication changes.

    Knowledge of use of blood sugar monitor devices.

    Report any alleged adult abuse to the Administrator. Report necessary information to the Administrator.

    Answer phone and take messages.

    Keep utility rooms neat and orderly.

    Fill out transfer sheets, make copies of med sheets, living will and POA for health care.

    Provide medication reminders per physician orders, routine and PRN’s. This includes oral, rectal, subcutaneous injection, IM injections, eye drops and ear drops and inhalers as determined in the tenants service plan. If client unable, then arrange home health with IDT input.

    Coordinate with home health agency for treatments as ordered by physician as determined in the tenants service plan.

    Other duties assigned by the Administrator and Corporate Nurse.
