Position: Clinical Consultant for HCM, Case Management Operations (CarePort Implementation)
6-month duration with a pay rate of $55/hr; The successful candidate will travel to various HCA facilities. There will be no patient contact but must have NURSYS
The Clinical Consultant for HCM, Case Management Operations will play a pivotal role in the transition to the CarePort system implementation. This position is a clinical subject matter expert dedicated to ensuring the successful deployment and optimization of case management technology. The consultant will provide critical clinical system guidance to execute Case Management workflow efficiencies and optimize new technology. A primary responsibility will be identifying current gaps and pain points to ensure the CarePort system enhances workflow efficiency and meets user needs.
The Clinical Consultant will be a key member of the implementation team, collaborating directly with the Hospital Case Management Director and Program Director, CMBT. The goal is to ensure seamless system design, development, and implementation while aligning the build with the current and future needs of the hospital's case management team.
Must-Haves:
- Experience with CarePort: Familiarity with CarePort system implementation and optimization is essential to ensure the successful adoption and integration of this technology.
- Registered Nurse (RN) License: An active RN license in the state of practice is required to provide the clinical expertise needed for case management technology deployment.
- Hospital Case Management Experience: A minimum of 3 years of experience working in hospital case management is required to effectively guide workflow improvements and align system features with clinical operations.
Major Responsibilities:
- System Assessment & Compliance: Continuously assess system standards, processes, and practices to ensure the Case Management technology supports industry best practices and complies with regulatory requirements.
- Clinical Expertise: Serve as a clinical subject matter expert, leading the implementation team in designing and developing system standards that meet case management needs.
- Optimization: Collaborate with department-wide teams to identify best practices and workflow optimization opportunities.
- Leadership Collaboration: Work closely with the Hospital Case Management Director and Program Director, CMBT, to ensure seamless system design, development, and implementation.
- Identify Inefficiencies: Proactively identify workflow inefficiencies and recommend solutions to optimize the case management process.
- Business Requirement Capture: Document and capture business requirements from case management leadership to ensure the system build meets both current and future needs.
- Stakeholder Engagement: Partner with key stakeholders, including CMBT, ITG, and Education teams, to support project success.
- Project Oversight: Assist with project management to ensure all considerations are built into enterprise-wide project plans.
- Risk Escalation: Identify and escalate any barriers or risks that may impact project timelines or success.
- Organizational Skills: Demonstrate strong organizational skills, proactively prioritize tasks, and manage resources effectively.
- Collaboration & Team Building: Foster team collaboration and support a positive working environment for case management staff.
- Future Automation Support: Assist with the integration of future technology automation partnerships and processes for Hospital Case Management.
Education & Experience:
- Bachelor’s degree in Nursing (RN) – Required
- Master’s degree in Nursing (RN) – Preferred
- 3+ years of experience with system application – Required
- 3+ years of experience in Hospital Case Management – Required
- Experience with CarePort – Required
- Experience with electronic medical records (Meditech, Epic, Cerner) – Required
Licenses, Certifications, & Training:
- Registered Nurse: Must hold an active RN license in the state of practice, in accordance with legal and regulatory requirements.
