Clinical Manager Transitional Age Youth (TAY) - (Part Time)
Job Description
Duties and Responsibilities:
Maintaining confidentiality of records relating to TAY’ treatment
Collecting information about TAY through interviews,evaluation and observation of day-to-day functioning within work, family and other routines.
Identifying areas needing continued support, resources and treatment in order to assist in TAY success.
Preparing and completing diagnostic and clinical assessment, clinical notes, referrals, treatment plans of TAY as appropriate on site and in the community as needed.
Working with the Clinical Team to ensure continuity of care for TAY.
Creating appropriate case management notes and input into systems as needed.
Facilitate TAY therapeutic workshops, presentations and groups as needed.
The TAY-CS coordinates entitlement services for REC clients in need of TAY-specific behavioral health treatment services to support the individual’s recovery efforts.
The TAY – CS ensures that TAY in need are connected to and receive support for mental health and substance use.
TAY- CS __ plan and host quarterly wellness workshops for REC staff and REC clients.
TAY-CS Collaborate regarding all activities and services offered to young adults;
TAY-CS Monitor and evaluate the timeliness and effectiveness of service delivery to REC
clients receiving services from the Transition Facilitators and /or Youth Development Leads;
Evaluate and analyze outcome data that tracks the number of referrals, successes/barriers to treatment supports, and programmatic updates.
All this must be accomplished and provided in compliance with the District of Columbia’s Mental Health Standards and regulations of MBI Health Services, LLC and all Federal, State and Local laws applicable to this agreement.
Salary : $56,600.00
