Great Lakes Integrated Network, IPA is looking for a Clinical Pharmacy Support Specialist to join our team!
The Clinical Pharmacy Support Specialist will play a key role in supporting the clinical pharmacy team by service as a liaison between patients, pharmacists, and external healthcare providers. The Clinical Pharmacy Support Specialist will work to ensure continuity of care for patients, and will serve as a care coordinator for patient medication and clinical pharmacy needs.
** Duties and Responsibilities (including but not limited to):**
** Under the supervision of a clinical pharmacist:**
- Answer incoming calls for the pharmacy department, providing excellent customer service and directing inquiries to the appropriate pharmacist or department.
- Communicate with patients, caregivers, and other healthcare providers to facilitate care coordination and address medication-related concerns.
- Manage the intake and processing of incoming patient referrals, ensuring timely scheduling of appointments, follow-up and other applicable patient related communication.
- Maintain accurate records of all referrals and outcomes in the electronic health record (EHR) system or other tracking tools.
- Coordinate care with pharmacists, other healthcare providers, and patients to optimize transitions of care and improve adherence.
- Obtain accurate and comprehensive medication histories for patients during transitions of care or as part of regular follow-ups, communicating discrepancies or concerns to the pharmacist for intervention and follow-up
- Assist patients in enrolling in patient assistance programs (PAPs) to ensure access to necessary medications.
- Manage and complete Durable Medical Equipment (DME) paperwork, working closely with insurance companies and suppliers to ensure timely delivery.
- Identify and coordinate care for patients who are non-adherent with their medications, working closely with pharmacists to develop and implement care plans.
- Proactively reach out to patients to remind them of upcoming appointments, prescription refills, and medication-related interventions.
- Accurately document all patient interactions, medication histories, and care coordination activities in the EHR, or other applicable platform.
- Ensure compliance with all legal and regulatory requirements related to patient privacy and medication management.
- All duties and responsibilities of the Clinical Pharmacy Support Specialist will comply with New York State law and the rules and regulations as promulgated by the New York State Board of Pharmacy.
- Other duties as assigned by the Director of Pharmacy Services, Clinical Pharmacy Manager, or Clinical Pharmacy Team Lead.
Qualifications for Education, Training and Experienc
Education (Minimum Needed):
Experience (Minimum Needed):
- 500 hours of pharmacy technician experience (within the past 24 months) OR
- Required completion of a nationally recognized pharmacy technician training program (ECC, BCAT, CPhT etc.)
Knowledge and Skills :
A foundational understanding of HEDIS and STAR quality measures is helpful for this position, although not required.

Pay range $19.00 to $26.00
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within job’s compensation range, and will be determined by considering factors including, but not limited to market data, education, experience, qualifications, and expertise of the individual, and internal equity considerations.
