    Location: Washington, DC

    Summary/Objective

    The Clinical Social Worker at Patient Centered Services will assist in providing psychosocial care and support to patients and their families. Though not licensed, the role involves conducting assessments, offering case management, and supporting licensed social workers in coordinating care. This individual will collaborate with interdisciplinary healthcare teams to address the mental, social, and emotional needs of patients in home health and community settings.

    Essential Functions

    Conduct initial psychosocial assessments under the supervision of a licensed social worker to determine patients' needs.

    Assist in developing and implementing care plans that address the psychosocial aspects of patients' health and well-being.

    Provide case management services, including coordination with community resources and support programs.

    Collaborate with nurses, physicians, and therapists to ensure holistic patient care.

    Participate in patient and family counseling sessions under the guidance of licensed social workers.

    Document patient interactions and assist in maintaining accurate records in the electronic medical record (EMR) system.

    Facilitate access to social services and community resources, ensuring that patients receive appropriate assistance.

    Attend multidisciplinary team meetings and contribute to the planning of patient care strategies.

    Assist in the development of patient and family support groups as needed.

    Ensure compliance with HIPAA regulations and organizational policies regarding patient confidentiality.

    Strong understanding of social work principles and ability to collaborate in a team environment.

    Effective communication skills, both verbal and written.

    Ability to manage multiple cases and tasks with strong organizational skills.

    Cultural competence and sensitivity when working with diverse populations.

    Basic case management and advocacy skills to support patients and families.

    Required Education and Experience

    Bachelor’s degree in Social Work (BSW) or related human services field from an accredited institution.

    Minimum of 1-2 years of experience in a social work or human services role, preferably in healthcare or home health settings.

    Preferred Education and Experience

    Master’s degree in Social Work (MSW) from an accredited program (in progress or pending licensure).

    Experience working with diverse populations, such as pediatric or geriatric patients.

    Multilingual abilities (especially English/Spanish) are preferred but not required.

    Additional Eligibility Qualifications

    Current CPR certification.

    Valid driver’s license and reliable transportation for home visits.
