    Summary:

    The clinical supervisor coordinates with all medical staff, to assist in the provision of professional medical care and clinical support within HAHC. A clinical supervisor plays a critical role in the delivery of quality patient care within the context of a Federally Qualified Health Center. They assist the Chief Medical Officer and Clinical Nurse Manager in planning, developing, evaluating and implementing all clinical services, ensuring quality patient care. They are responsible for efficient patient traffic flow and overall clinical efficiency and effectiveness.

    Duties, Functions, and Responsibilities:

    Assist with comprehensive patient examinations, screenings, health education, and treatment of patients within the scope of practice, using a systematic approach to history and physical appraisal of the patient.

    Provides appropriate counseling and health education to patients, providing necessary literature and instructions for continuity of care.

    Functions as a triage; interact with other agencies/medical entities in regards to patient medical information and documenting referrals, as necessary.

    Assist provider in screening patient health complaints by phone or in person. This requires clinical supervisor to work within in the scope of practice and triage comprehensive questions/concerns to the provider.

    Accurately prepares, administers, and records medications and/or procedures directed by the Chief Medical Officer.

    With direction from provider, will call patients to communicate lab results

    With direction from provider and per HAHC policies and procedures, will process medication refills.

    Instructs and counsels patients regarding compliance with prescribed therapeutic and drug regimens; documenting and maintain patient records of services provided according to prescribed program standards.

    Develops and implements patient comprehensive management plans, records progress notes, and assists in provision of continuity of care.

    Able to accurately execute technical clinical procedures when assigned.

    Able to maintain storage and handling of clinical equipment, supplies and drugs; ability to lift, carry and store supplies within the center as directed.

    Responsible for ensuring clinical policy manual and clinical compliance with all applicable federal, state and local rules and regulations governing primary health clinics and Federally Qualified Health Centers.

    Supports the Patient Centered Medical Home initiative as directed by the CQI Team.

    Responsible for selecting, orienting, training, and disciplining employees; communicating job expectations; planning, monitoring, appraising job contributions; adhering to policies and procedures.

    Oversee shift operations to ensure full coverage for all patients. Support patient care and treatment as needed.

    Other clinical duties and responsibilities as assigned by the Chief Medical Officer and Clinical Nurse Manager.

    General Duties:

    Maintains confidentiality of all health center related information within the bounds of patient and professional confidentiality.

    Consistently demonstrates job knowledge and skills to perform all job responsibilities effectively and professionally.

    Identifies problems, makes recommendations and assists in developing solutions to resolve concerns with co-workers professionally and constructively.

    Strives to improve operations, co-worker and teamwork within the department and with all other departments, Board and administration of health center.

    Encourages the mission and goals of the health center at all times.

    Ensures all policies and procedures are adhered to.

    Knowledge, Skills, and Abilities:

    Knowledge and skills for the provision of patient care in the practice of primary health care that is age and culturally appropriate.

    Knowledge of principles or growth and development of all life spans.

    Skill in communicating effectively with patients, their families and other staff members; interpreting patient data for care requirements after medical assessment.

    Skill in establishing and maintaining effective working relationships with other employees, patients and the general public.

    License or Certification:

    Certified Medical Assistant / Licensed Practical Nurse /Registered Nurse

    CPR/AED Certification
