    The QMAP/Med Tech is responsible for assisting in the administration of medications to residents as ordered by the attending physician under the direction of the attending physician, the nurse consultant, and the Director of Clinical Services. The administration of medications shall be in accordance with established nursing standards; the policies, procedures, and practices of the community; and the requirements of the state. In addition, the QMAP/Med Tech will provide direct hands on care in accordance with residents'service plans, company policies and procedures, and as may be directed by the supervisor. The ideal QMAP/Med Tech candidate must have a high school diploma/GED and one year of previous assisted living/nursing experience with direct resident care is preferred, but not required. In addition, he/she must have received the medication training by an RN and observation/evaluation of the ability to perform safe medication and treatment administration must be documented.

    As a QMAP/Med Tech, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.     RESPONSIBILITIES

    * Supports, follows, and carries out the company vision, mission statement, and values

    * Upholds confidentiality and resident privacy at all times (adheres to HIPPA of 1996)

    * Reports abuse/ neglect and grievances by filling out an incident report, alerting the immediate supervisor, and/or calling the Ombudsman

    * Records food intake and body output as required by service plan

    * Reads and makes entries in notebook/communication log and 24 hour book to track resident status, daily needs, or changes in services from previous shift

    * Observes, records, and documents changes in writing on the Service Plan, 24 Hour Report, ADL flow sheet, Communication book, and resident chart

    * Assists with the admission of new residents. (i.e., listing personal belongings, introducing to other residents, etc.)

    * Assists resident with bowel/bladder needs and provides pericare according to residents'preferences and with the use of condom catheters and daily catheter care

    * Keeps residents clean and dry (i.e. change gown, clothing, linen, etc., when it becomes wet or soiled) and assists resident with getting dressed and undressed

    * Assists with resident bathing in a tub/shower or with sponge bath

    * Assists with special skin care to prevent decubitus ulcers and turns resident in bed according to proper procedures to prevent sores

    * Assists resident in physical activity, including walking, standing, transferring, and passive range of motion exercises

    * Provides support to resident by responding to requests for lifting, transferring, and escorting and redirects residents who are wandering

    * Assists resident with grooming, including shaving, combing hair, and nail care (non-diabetic)

    * Provides transport support by using multiple person lifting techniques and provides transfer support using the Hoyer lift and the Gait Belt

    * Assists residents with the use of elastic stockings, eye glasses, hearing aids, teeth brushing, and denture care

    * Make occupied or unoccupied beds, changes bed linens, and keeps linens tight to avoid wrinkles from forming under the resident

    * Assists resident with gathering, loading, folding, and putting away of laundry and with care of personal belongings by placing items in designated areas

    * Provides reading support to resident by reading letters, medical bill/information, and calendars

    * Promotes resident independence by encouraging them to remain independent in the areas where they are self sufficient

    * Accurately measures and records vitals as required, including temperature, oxygen saturation, weight, pulse, respiration, and blood pressure

    * Assists resident with changing bandages or dressings as needed

    * Collects routine urine, stool, and sputum specimens according to proper procedures

    * Maintains the confidentiality of all resident care information including protected health information and reports known or suspected incidents of unauthorized disclosure of such information

    * Performs all assigned tasks in accordance with our established policies and procedures and as instructed by supervisors

    * Assists in orientation and training of new staff when necessary

    * Accurately and safely prepares, administers, and documents the oral and/or topical medications ordered for resident use by the attending physician

    * Verifies that any medications brought into the community by a newly admitted resident are examined and identified by the attending physician or the community pharmacy or pharmacist

    * Follows the labeling policies and practices of the community and ensures that all medications administered are properly labeled

    * Follows community policies and procedures regarding the destruction of medications and assists in carrying out these rules under the direction of the Administrator, Resident Care Coordinator, RN Consultant, and/or Pharmacy, including documentation requirements

    * Assists, as directed, in documenting and removing medications that are discontinued by the attending physician

    * Follows community procedures governing drug receipt errors, including notifying the delivery source and the Resident Care Coordinator

    * Ensures that documentation of the drug receipt error includes a written error report, signed by self and an observer/witness

    * Keeps medication cart clean and orderly, removes expired drugs, and keeps medication cart locked when not in view or unattended

    * Follows community procedures in holding medications for residents who are transferred from the community or when the attending physician discontinues medications

    * Observes and verifies that medication is ingested or applied as directed and documents any instance where prescribed medication is not administered, including reason for refusal

    * Ensures that no medication is administered without a written, signed, dated order from an attending licensed physician

    * Verifies the identity of the resident before administering the medication treatment

    * Follows the community's procedures allowing residents to self-administer his or her medications

    * Follows swallow precautions for all residents who are identified as having swallowing problems

    * Encourages resident in nutritional needs by encouraging a balanced diet or adherence to a special diet

    * Prepares residents for meals (i.e., takes to bathroom, washes hands, combs hair, positions tables, places bibs, takes to/from dining room, etc.)

    * Serves and cleans up after meals and serves food trays as requested

    * Assists with feeding as indicated (i.e., cutting foods, feeding, assisting in dining room supervision, etc.)

    * Performs routine daily housekeeping duties and keeps rooms, bedside tables, closets, and drawers orderly

    * Attends and participates in training/in-services/continuing education programs to stay abreast of changes in profession and to maintain the required number of annual CEUs

    * Follows established safety regulations, including, but not limited to, fire protection/ prevention, smoking regulations, infection control, medical devices, etc.

    * Practices safety and knows emergency procedures and the policies and procedures for dealing with bodily fluids, hazardous waste, and infection control

    * Performs other duties as assigned

    QUALIFICATIONS

    * High school diploma/GED with further documented medication training

    * Must be 18 years of age or older

    * Must be a certified QMAP and be current in the state of Colorado

    * Must have completed medication training by an RN and documented observation and evaluation of the ability to perform safe medication and treatment administration

    * Able to follow spoken and written instructions

    * Able to maintain general sanitary/safety practices

    * Genuine desire to work with the elderly and/or disabled

    * Must be able to respond appropriately to emergency situations

    * Must have patience, tact, a cheerful disposition, and enthusiasm and the willingness to deal with personnel, community staff, visitors, government agencies/personnel, vendors, and the general public

    * Able to bend, kneel, squat, stand, and lift up to 75 pounds as needed for this job
