POSITION SUMMARY
Provides companion and personal care services under the direction of the Registered Nurse and Scheduler. The Home Health Aide is assigned to specific clients by the Scheduler performs services for clients as necessary to maintain their personal comfort.
REPORTS TO: Scheduler / Registered Nurse
QUALIFICATIONS
Successful completion of a formal certification training program and/or a written skills test and competency evaluation.
Be at least eighteen (18) years of age.
Be able to provide a negative tuberculosis test.
Successful completion of a Level 2 Background Check
Demonstrated ability to read, write, and follow a written Plan of Care.
Good verbal, written, and interpersonal skills.
Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Must have a good driving record and valid driver's license.
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
Performs meaningful activities with clients to bring them joy and improve their quality of life.
Word and board games
Community outings and activities
Art and music activities
Stimulation through smells, taste, and touch
Performs personal care activities (ADLs), including but not limited to:
Bathing
Shampooing
Skin care/nail care
Oral hygiene
Shaving
Dressing
Performs household services essential to health care at home, including but not limited to:
Meal preparation/feeding
Laundry
Light housekeeping
Assists clients with self-administration of medication.
Provide transportation for clients to and from appointments/ errands.
Reports any observed or reported changes in the client's condition and/or needs to the Registered Nurse. Follows reporting guidelines in the Care Plan.
Documents care provided in the caregiver phone application.
Promotes personal safety and a safe environment for clients by observing infection control practices, following ComForCare policies and guidelines, and reporting unsafe situations to the Supervisor.
Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.
Communicates effectively with the office staff.
Maintains confidentiality in all aspects of the job.
Completes required online trainings (CEUs) to meet compliance requirements. Identifies learning needs to the supervisor.
Performs other related duties and responsibilities as deemed necessary.

Lift, reposition, and transfer clients.
Sustain weight of clients using proper transfer safety techniques and equipment (example: use of transfer belt while assisting client to transfer from bed to chair).
Travel to client's place of residence using a reliable automobile with current insurance coverage and a valid driver's license.
Be willing and able to travel to the following counties to provide care when needed: Duval, Clay, St Johns, Nassau.
