We are seeking a Community Care Manager who will help manage business operations at our satellite offices. This position is expected to work full-time and will travel between sites as needed. The ideal candidate is proactive in networking, passionate about delivering quality care, and dedicated to fostering business growth with compassion.
RESPONSIBILITIES:
Serve as a liaison between CarePlus and partner communities, ensuring seamless communication about operations and client concerns
Build and maintain strong relationships with residents, staff, and management to drive retention and business growth
Organize and assist with residential activities and social events to enhance engagement and promote CarePlus services
Conduct regular follow-up calls and visits with clients to ensure service satisfaction
Address and resolve client issues promptly and effectively
Oversee general office operations, including filing, inventory management, and ordering supplies
Maintain and update WellSky, ensuring accurate client records and scheduling information
Oversee census updates, including admissions, discharges, and status changes
Manage incoming phone calls during business hours and take after-hours calls on a PRN basis
Provide backup coverage as needed for cases at satellite locations
Design and distribute promotional materials, targeting a 20% growth in services
Travel between CarePlus offices as needed and participate in routine huddles and meetings
QUALIFICATIONS:
Active CNA and CMT licenses
High school diploma or equivalent
Professional maturity and ability to manage job demands effectively
Team-oriented and able to thrive in a fast-paced environment
Passion for providing excellent service and assisting others
Excellent interpersonal, communication, and problem-solving skills
Affinity for business development and marketing
Strong organizational skills with attention to detail and a proactive mindset
Basic understanding of medical terminology
Proficiency in computer systems and ability to learn new software
At least 1 year of experience in a coordinator, scheduler, or similar support role preferred
