Community Medical Center is currently accepting applications for a full-time Community Health Navigator. The Community Health Navigator will serve as a primary point of contact for patients who need assistance in understanding their health insurance options, accessing coverage, and enrollment. This position will work closely with patients and their family members to provide care management, care coordination, and outreach services. The Community Health Navigator will be knowledgeable about community resources appropriate to the needs of patients/families and assist patients with completing applications and registration forms. Responsible for scheduling and transporting (up to 50 miles one way) or arranging for transportation through CMC transportation. This includes driving patients.

Qualifications: High School Diploma or GED required, bachelor's degree in related healthcare field is preferred. Effective and proven communication skills, compassion, empathy, problem-solving skills, and the ability to multi-task are required skills all candidates must exhibit. Prior experience working with hospital or nursing-home patients is preferred.

This position will be required to complete the Community Health Worker certification within the first 12 months of employment.
