    Description:

     

    Work location - UnitedHealthcare Community Plan of North Carolina Community and State telecommute position. Lives in NC is preferred.

    Position background ? What does this position accomplish for the business? Conduct member outreach to UnitedHealthcare Community Plan of North Carolina members who are overdue for preventive care services and screenings. Engagement with members, parents or guardians to address any social determinants of health, connection to resources and primary care providers promoting healthy outcomes for our members.

    ? What are the top 5-10 responsibilities for this position? Conduct screenings of member health care needs. Work with members to develop healthcare goals and identify potential barriers to achieving those goals. Utilization of motivational interviewing techniques to help member identify and understand their intrinsic goals and motivate member to engage in positive behavior change. Coordinate member access to health-related programs, services and/or providers. Coordinate with health care providers to facilitate member care and/or services. Provide education to members on health and wellness information, member plan benefits and how to access those benefits, and appropriate utilization of services based on need. Review member health-related information using internal and external systems. Use desk top computer applications (Microsoft Excel, Word, Outlook) to summarize information or communicate with others. Execute and meet performance goals within a virtual environment.

    What does the ideal candidate background look like? Experience within the healthcare industry. Demonstrates a moderate level of knowledge and skills in own function. Solves routine problems on own. Works with supervisor to solve more complex problems. Prioritizes and organizes own work to meet agreed upon deadlines and works well with others as part of a team.

    What skills/attributes are required (please be detailed as to number of years of experience for each skill)? Required high school education or equivalent experience. Proficiency in Microsoft applications including but not limited to Excel, Word, Outlook, Teams, and PowerPoint.

    What skills/attributes are preferred (what will set a candidate apart)? Bachelor's degree in social work, licensed practical nurse (LPN), or high school graduate with three years of experience working within the community health setting in a health care role. Role does not write care plans.

    Does this position require a professional license or certification? No.

    What are the work hours (ex. 9am-5pm day/night shifts rotating shifts etc.)? Monday through Friday 8am - 5pm
