    Position Summary

    The Community Health Worker (CHW) plays a vital role in bridging the gap between healthcare providers and community members. CHWs assist patients and their families in navigating and accessing essential community services, adopting healthier behaviors, and connecting with resources that improve their quality of life. This role supports integrated care management and outreach initiatives to promote, maintain, and improve the overall health and well-being of individuals and families served by the EFC.

    Essential Duties and Responsibilities • Build trusting relationships with patients and families, offering support and encouragement. • Conduct follow-ups with patients via phone calls, home visits, or in other relevant settings. • Assist patients in setting personal health-related goals and attending appointments. • Provide referrals to appropriate community agencies and resources. • Facilitate patient access to transportation and provide appointment reminders as needed. • Act as a liaison between patients, providers, and community service agencies. • Collaborate with medical providers and case managers to ensure coordinated and comprehensive care plans. • Offer social support, advocacy, and informal counseling. • Maintain accurate documentation in the Electronic Medical Record (EMR) within 24 hours of patient contact. • Deliver services such as basic first aid, blood pressure screenings, and simple wound care. • Attend staff meetings, trainings, and fulfill other organizational responsibilities as assigned.

    Physical Requirements • Ability to sit, stand, and walk for extended periods. • Occasionally lift or move up to 25 pounds. • Maintain focus in a busy, fast-paced office environment.

    Qualifications

    Education and Training: • High school diploma or equivalent required. • Completion of a Community Health Worker training program (preferred). • Medical training or certification such as EMT, Paramedic, or CNA is a plus.

    Experience: • Background in medical terminology or healthcare settings is advantageous. • Previous experience in community outreach, case management, or related roles preferred.

    Skills: • Proficiency in motivational interviewing and goal setting. • Familiarity with community resources. • Basic computer and EMR documentation skills.
