    Worcester RISE for Health is seeking a passionate and dedicated Community Health Worker to join our team. This position would be dedicated to supporting newly arriving refugee and migrant families. The focus will be on supporting pregnant individuals and infants under 5. The Community Health Worker will play a crucial role in improving the health and well-being of these families. They will work closely with pregnant individuals and families to provide support, education, and resources to access healthcare services and other benefits that will help them have a safe and healthy delivery and support their thriving infant. • For this specific position must speak Haitian-Creole • Position is Part-time with the opportunity for growth to full-time, minimum 24hrs/wk, up to 30hrs/wk Responsibilities: Outreach and Engagement: • Identify and reach out to pregnant individuals and families in the community who may benefit from health navigation support and connecting to resources • Build trusting relationships with community members to facilitate open communication Assessment and Education: • Conduct health assessments to understand the unique needs and challenges of community members • Provide education on various health topics around prenatal and newborn care Resource Navigation: • Assist individuals in navigating the healthcare system, including addressing health insurance challenges, connecting to primary care practices, identifying transportation options, and assisting in navigation during appointments • Connect clients to social services and resources to address social determinants of health Health Promotion: • Organize and participate in health promotion activities and events within the community • Collaborate with local organizations to develop and implement health-related programs Data Collection and Reporting: • Maintain accurate records of interactions and services provided • Compile data and generate reports to track the progress and outcomes of community health initiatives Qualifications: • Associated or Bachelor's degree in a related field • Previous experience in community health, social work, or related fields is preferred • Passion for maternal child health • Strong communication and interpersonal skills • Cultural sensitivity and the ability to work with diverse populations • Knowledge of local healthcare resources and services • Ability to work independently and as part of a team • Proficiency in basic computer skills – specifically Word, Excel, and Email • Proficiency in Haitian-Creole Compensation: $20+ hourly based on experience
