    Position Summary

    Saving Hopes of RI is seeking a compassionate and community-driven Community Health Worker (CHW) to serve as a trusted connector between individuals and local health and social services. The CHW will play a key role in supporting underserved populations through outreach, education, advocacy, and navigation of resources to improve health outcomes and overall well-being.

    Key Responsibilities

    Conduct community outreach to identify individuals and families in need of support.

    Build relationships with community members through regular engagement, home visits, or events.

    Provide culturally appropriate health education and resource information.

    Assist clients with accessing services such as healthcare, housing, food assistance, and mental health care.

    Advocate on behalf of clients and support them in navigating systems (e.g., healthcare, housing, social services).

    Collect and report data to track client needs, services provided, and outcomes.

    Collaborate with healthcare providers, social workers, and other community-based organizations.

    Participate in training, staff meetings, and community events as required.

    Qualifications

    High school diploma or GED required; some college or training in public health, social work, or a related field is preferred.

    Experience working in community-based settings or lived experience within the community served is highly valued.

    Must complete and pass an approved Community Health Worker (CHW) training program within 18 months of hire.

    Excellent interpersonal, communication, and problem-solving skills.

    Ability to maintain confidentiality and demonstrate cultural sensitivity.

    Must be self-motivated, organized, and able to work independently.

    Comfortable with basic data entry and use of mobile apps or reporting tools.

    Valid driver’s license and reliable transportation (if travel is required).

    Work Conditions

    Some local travel may be required within Rhode Island.

    Occasional evening or weekend work depending on community events or client needs.

    Saving Hopes of RI does not currently offer retirement or health benefits.

    $20.00/hour (based on experience and funding)
