    Job Title: Master Social Worker (MSW)

    Position: Community Integration Counseling (CIC)

    Location: New York

    Hour: Per diem

    Hourly Rate: $30-35 per hour

    home healthcare service provider specializing in waiver services under the New

    York Department of Health's Nursing Home Transition and Diversion (NHTD)

    program and Traumatic Brain Injury (TBI) program. We are committed to

    Position Overview:

    We are seeking a dedicated MSW (Master of Social Work) to join our team and

    provide Community Integration Counseling (CIC) Services. The MSW will work with

    individuals directly to develop and implement person-centered plans that support

    the individual's goals and preferences, and promote independence and community

    integration.

    Responsibilities:

    ● Conducting an initial assessment to understand the individual's strengths,

    needs, and goals for community integration

    ● Developing a person-centered plan that outlines the support needed to help

    the individual engage more fully in their community and improve their overall

    quality of life.● Providing counseling services to help individuals manage mental health

    conditions, behavioral challenges, or other issues that may hinder community

    involvement.

    ● Life skills training, such as social skills, communication skills, and

    problem-solving techniques, to help individuals engage effectively in community

    settings.

    ● Connecting individuals with local community resources, such as vocational

    programs, recreational activities, educational opportunities, or social groups.

    ● Encouraging participation in community events and activities, fostering

    relationships, and helping individuals navigate their environments in a way that

    promotes independence.

    ● Facilitating access to vocational training or supported employment opportunities

    that help individuals enter or re-enter the workforce.

    ● Advocating on behalf of individuals to ensure they have access to the services,

    programs, and support they need in the community.

    ● Supporting family members or caregivers with information and resources to help

    them provide effective care and encouragement.

    ● Developing and implementing crisis intervention strategies when needed,

    ensuring that individuals have the support they need to stay safe and continue

    progressing.

    ● Referring clients to other specialized services as needed, such as mental health

    treatment, addiction services, or specialized rehabilitation.

    ● Maintaining thorough records of client progress, goals, and service plans to

    ensure compliance with New York State regulations.Qualifications:

    ● Bachelor's degree in social work, psychology, or a related field.

    ● Two (2) years of experience providing adjustment related to counseling to

    seniors and/or individuals with physical and/or cognitive disabilities and

    their families to be considered qualifying experience.

    ● Knowledge of the NHTD/TBI program and regulations.

    ● Strong communication and interpersonal skills.

    ● Ability to work independently and as part of a team.

    ● Proficiency in computer skills, including electronic health record

    Systems. Valid driver's license and reliable transportation.
