Qualifications

At least one year of experience working with elders or adults with disabilities.

Ability to apply unique understanding of the experience, language, or culture of the populations serve.

Must live in Massachusetts

Responsibilities The responsibilities of the Community Liaison include:

Assist in promoting the agency and fostering community relations

Apply unique understanding of the experience, language, or culture of the populations serve.

Assists in the evaluation of community needs and services to meet with identified needs.

Acts as liaison with member/caregiver and the agency

Establishes and maintains effective channels of communication.

Providing to the member and caregiver culturally appropriate health education, information, support, and counseling.

Participating in the development, implementation, and ongoing review of the AFC plan of care

Conducting on-site visits with the member at the qualified setting

Assisting with obtaining information and accessing other health-care and community Services and making referrals to appropriate service provider s if the member requires services other than those provided by the AFC provider in conjunction with RN.
