    Position Description:

    The Navigators will be responsible for recruiting young adults to enroll in the Service Connector and Navigation (SCaN) program, providing case management services to young adults ages 18-24 to assist them in providing stability to their lives by: working with them to create a service plan to include goal setting, connecting them to services that support their goals and to meet other needs, conducting outreach services to recruit participants for SCaN program.

    Reporting:

    Reports to the Program Manager

    Duties and Responsibilities:

    The Navigator will:

    • Participate in organization trainings and workshops

    • Be assigned a caseload of 15 youth to provide support services

    • Act as an advocate for the youth on their caseload to ensure they receive the

    needed care and support to complete the SCaN program

    • Work directly with young adults to address barriers such as transportation,

    Childcare, etc.

    • Create goal and service plans with each client

    • Create transition plans for youth as they complete goals and prepare to exit the

    program

    • Accompany young adults to program trainings (i.e. job readiness, life skills,

    financial literacy)

    • Monitor participants progress and adjust goals and schedules accordingly

    • Provide youth with gift cards as they reach goals and objectives created in service

    plans

    Qualifications:

    • Experience in urban educational or working with community-based settings,

    serving low income and minority students

    • Must be able to pass background check

    • Ability to create the best conditions for learning or teaching new things

    • Excellent Leadership skills.

    • Relate well to young adults

    • Passionate about the helping others succeed

    • Passionate about serving others

    • Flexible and open to change.

    • Excellent verbal communication skills.

    • Good computer skills (Micros soft EXCEL and Word)
