    POSITION TITLE: Community Resources/Transportation Aide

    Overview of the Role

    The Community Resource Aide is essentially responsible for recording social isolation data established by maintaining the engagement of the DACL network and targeting the elderly population who are at risk for social isolation and food insecurity. The Community Resource Aide will additionally provide coordination of activities through the use of Seabury DC transportation and other transportation vendors as needed.

    This position operates as a part of the social innovation team. This department manages Community Dining Sites and provides ongoing social and recreational activities to older adults.

    Responsibilities

    Major Responsibilities/Activities:

    ● Access online program resources that are relevant to the interests and accessibility of the program participants.

    ● Assists with conducting public meetings with stakeholders, which includes sharing information about transportation and other program services provided by Seabury Resources for Aging..

    ● Develop community activity and transportation calendars for Seabury Resources for Aging., Wards 5 & 6.

    ● Provide documentation to the Transportation Coordinator to support transportation billing.

    ● Communicates information to residents and other stakeholders on various community issues.

    ● Coordinates and schedules outreach activities, including distribution of information to residents.

    ● Research, curate, and distribute articles related to social isolation and food insecurity to encourage and support participants.

    ● Maintains documentation of engagement with participants in CSTARS.

    ● Compiles routine data to support activities and outreach goals to provide to management upon request

    ● Follow-up with potential new participants and stakeholders who are seeking services.

    ● Ensure the resources are accurately stated and conveyed to customers

    ● Meet all deadlines for reporting service units according to the set schedule that reflects your effort at engaging participants and community stakeholders on a daily and/or routine basis.

    ● Conduct Individual Socialization calls on clients utilizing the Seabury Connector transportation services.

    ● Increase and expand the transportation service delivery through outreach and community engagement.

    ● Review DACL Client Intake and Nutrition Screen Forms to ensure completion of the required information to provide services.

    ● Maintain accurate and current documentation on customers utilizing Seabury DC transportation services.

    ● Assist with the development and completion of a monthly calendar for the Community Dining Sites.

    ● Serves as a backup driver for transportation programs driving a 12+2 vehicle to events and monthly grocery trips.

    ● Manage and develop invoices for DACL Transportation program

    ● Perform other duties as assigned.

    Key Qualifications

    Possession of high school diploma, GED, or equivalent experience

    Data entry experience

    Human Service background

    Proficient with Microsoft Suites Application

    Must be knowledgeable of issues and challenges that impact older adults.

    Must be sensitive, courteous, and respectful to the needs of older adult participants.

    Salary: $40,000 - $45,344/Annually
