    Reach7 Diabetes Studios is a membership-based, concierge clinic specializing in diabetes, prediabetes, and weight loss. Located conveniently in shopping centers next to Publix pharmacy and grocery stores, we are striving to bring the first brand of "The Neighborhood Diabetes Provider" by improving patient access, maximizing convenience, and increasing collaboration with the local community's health resources.

    Overview

    We are developing a studio in the Newnan, GA region and are looking for highly qualified and truly compassionate nurse practitioners / physician assistants to anchor this location. This position is ready to start as soon as late 2024 / early 2025.

    Job Description

    • Evaluate, diagnose, and treat patients with all types of diabetes (Type 1, Type 2, gestational), prediabetes, weight management issues, and other related comorbidities.
    • Conduct patient interviews and collect patient histories, psychosocial histories, health status, and medical/potential problems.
    • Order, schedule, and/or perform laboratory tests and diagnostic procedures such as blood tests and urinalyses. Collect and interpret diagnostic data to implement appropriate patient care plans.
    • Provide behavioral health coaching and educate patients on health habits, self-monitoring, nutrition, physical activity and movement, sleep, stress, at-home medical care, medications, and other issues related to their medical or health problems.
    • Maintain and update current, accurate patient records including medical assessment, interventions, and evaluations.
    • Provide updates on patients' health status and progress to patients, patients' family members, and/or designated caregivers as appropriate.
    • Participate in community outreach and collaborate with local healthcare professionals for any specific patient medical needs, including pharmacy, primary care, cardiology, nephrology, neurology, optometry, podiatry, and other specialty providers.
    • Stay abreast of and excel at utilizing the latest therapies, technologies, and best practices in all types of diabetes (Type 1, Type 2, gestational), prediabetes, weight management, and other related comorbidities. Maintain expertise in clinical guidelines such as ADA, AACE, AHA, ACC, AND, and KDIGO.
    • Maintain technical expertise in digital solutions, including all administrator, provider, and patient-facing applications, clinical decision support tools, externally integrated solutions, supported medical devices, electronic health records, scheduling tools, and newer AI-enabled solutions.
    • Collaborate with other team members to regularly evaluate, refine, and improve processes in the clinics, tools being used, and patient experience.
    • Assist with the screening and hiring of new team members as needed.
    • Actively engage with marketing efforts, which may include participation at events, social media, radio, and TV.

    Qualifications

    • Completion of an accredited nurse practitioner or physician assistant program.
    • Currently licensed as an advanced practice nurse in the state of Georgia by the Georgia Board of Nursing and certified as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC) or equivalent body, or Certified as a Physician's Assistant by the National Commission on Certification of Physician Assistants (NCCPA).
    • Licensed as appropriate in the state of Georgia.
    • Two (2) or more years of experience managing patients with diabetes and/or prediabetes in a clinical setting.
    • Exceptional leadership, organizational, communication, and interpersonal skills.
