The Contract Registered Nurse (RN) will oversee initial assessments, care plan approvals, and supervisory visits.
Compensation:
Initial Assessments and Care Plan Development: $150 to $200 per visit.
Supervisory Visits: $100 to $150 per visit.
Care Plan Updates (without in-person visits): $75 to $100 per update.
Travel Compensation: Offer mileage reimbursement at the IRS standard rate (e.g., $0.655 per mile for 2024
Key Responsibilities:
Clinical Assessments and Care Planning:
Conduct initial assessments to create individualized plans of care for Veterans.
Approve initial and updated care plans within required timeframes.
Evaluate Veterans' physical, cognitive, and emotional needs, ensuring services align with VA authorization.
Supervisory Visits:
Perform required supervisory visits every six months for all PCS services, as mandated by VA regulations.
Monitor caregiver performance to ensure adherence to care plans.
Document and submit supervisory visit reports to the VA on time.
Quality Assurance:
Ensure compliance with VA policies and clinical standards.
Provide clinical oversight and training recommendations for caregivers.
Identify and address any gaps or issues in service delivery.
Documentation and Reporting:
Complete and submit all required documentation, including Supervisory Visit Forms and updated care plans, securely and within deadlines.
Report significant incidents, service holds, or changes in Veteran status to the VA staff.
Coordination and Communication:
Liaise with agency staff, VA caseworkers, and family members regarding care plans and service delivery.
Serve as the point of contact for clinical concerns related to PCS.
Qualifications:
Education: Bachelor's or Associate Degree in Nursing from an accredited program.
Licensure: Active and unrestricted RN license in Texas.
Experience:
Minimum of 2 years of clinical experience in a healthcare or home care setting.
Previous experience working with Veterans or in a Personal Care Services program is a plus.
Skills:
Strong assessment, care planning, and documentation skills.
Proficient in secure communication methods (e.g., encrypted email).
Excellent interpersonal and communication skills.
Knowledge of VA regulations and compliance requirements is preferred.
