C.O.R.E. Medical Clinic, Inc. is taking over a contract that provides services to incarcerated patients in Main Jail and RCCC. Office location and exact duties are to be determined but a basic job description is below. You must be comfortable being dually supervised by CORE and Sac County Jail. Hours will be 8am-4:30pm with a 30-minute lunch.

Conducts patient intakes and assessments with treatment presented in a positive way. Development of comprehensive, individualized treatment plans reflecting the issues and needs of each patient identified during the assessment process and on an ongoing basis
Counsel patients one-on-one based on areas of need identified in the treatment plan.
Utilizes evidence-based counseling techniques including Motivational Interviewing and/or Cognitive Behavioral Therapy. Maintains patient records in accordance with all applicable Federal, State, County and C.O.R.E. requirements.
Meets C.O.R.E. standard for counseling time?
Functions as a positive, stable role model, demonstrating concern for patients.
Participates in multidisciplinary treatment team staff meetings and coordinates care with other team members.
Facilitates groups and educational sessions as needed, utilizing SUD curriculum that is evidence-based, supp.
Provides Correctional Health staff education and/or consultation.
Adheres to Correctional Health policies and procedures and SSD Custody safety and security protocols as directed.
Performs related work as required.
Consult with jail medical staff on psych/mental health issues
Training and Experience
Candidates for this position must be certified or registered as a chemical dependency counselor in the State of California. Minimum 2 years SUD experience and working in a correctional setting is preferred. The counselor must be able to work in various situations subject to defined policies, procedures, and practices. The candidate must possess demonstrated assessment, problem solving, and counseling skills in the area of substance abuse treatment. The candidate must pass an initial security clearance and maintain an active security clearance at all times.
