The counselor will be responsible for meeting with identified individuals on a weekly basis and providing ongoing counseling. In addition, the counselor will work with the Behavioral Specialist and Gracedale staff to successfully implement behavioral plans. They can also provide minimal counseling to other qualified individuals needing some support. The counselor will work in unison with Gracedale staff on ways to implement the plan to address problematic behavior.
Provide behavioral interventions as identified in the behavioral plan developed by Specialist
Discuss progress of behavioral plan and make changes where necessary and address problem areas
Individual counseling with identified individuals
Updating the team on any issues or concerns with the identified individual, as well as positive steps taken.
Carry out agreed upon behavioral and treatment plans for identified individuals
Confirm eligibility with the county prior to services
Complete supervision with Behavioral Specialist to discuss cases and progress of behavioral plans
Maintain detailed session notes for each session
Attend supervision, team meetings and agency staff meetings
Attend required trainings
Other duties as assigned
Position requires a bachelor’s degree in social work, Psychology, Counseling or related field. Minimum of one year experience with individuals diagnosed with a mental illness. Must complete all required background checks and screenings.
