DDA Program Coordinator - Salisbury
Certified Home Nursing Solutions is looking to recruit a qualified Program Coordinator. Qualified applicants must be dependable, detail oriented, an effective communicator, and able to supervise direct support professionals.
The Program Coordinator position will oversee all aspects of the Person-Centered Planning process from summary development to oversight of implementation and data collection, assisting with staffing needs, and communication with staff and administrative staff. This position also is responsible for scheduling staff for assigned shifts and duties. The Program Coordinator is part of a team responsible for assisting each individual meet their desires and preferences in regard to building their individualized daily schedule whether it consist of pre-vocational training or social and leisure activities, in a safe, enjoyable environment. Attend staff meetings/trainings included but not limited to DDA required trainings, CPR/First Aid, MANDT, and CMT.
- JOB RESPONSIBILITIES:
- Oversee a caseload of individuals supported and complete required documentation in a timely manner – SIP, Quarterlies, DSAT and Cost Detail. Attend staff meetings/trainings included but not limited to DDA required trainings, CPR/First Aid, MANDT, and CMT.
- Prepare required documentation and distribute to appropriate locations including uploading to WellSky.
- Review and monitor data collection for accuracy and implementation strategies.
- Assist with the writing of goal implementation strategies and task analysis
- Attend PCP meetings and other team meetings as necessary.
- Oversee, produce, and distribute a monthly program calendar that is tailored towards leisure preferences of those we support.
- Develop daily staff schedules/assignments.
- Assist individuals with activities of daily living as coverage is needed
- Assist and oversee individualized schedules of those supported.
- Report programmatic QA reports/incidents to the Meaningful Day Services Manager
- Process petty cash requests from DSPs for incidental purchases
- Assist with staff development, orientation, and training
- On-Call Rotation
Qualifications:
- RN License, Bachelor or Associate Degree
- Three years (3) of experience working with people with developmental disabilities or special needs in a day program setting or similar.
- Strong office organizational skills and proficiency in Microsoft Word, Excel and LTSS/PCIS2
- Scheduling experience in the health care industry
