Licensed Vocational Field Nurse (LVN) Home Health

Desire Home Health Care Agency is currently seeking experienced and dedicated Licensed Vocational Field Nurses to join our Home Health team in the Inland Empire Area. The skilled nurse will be able to make intermittent visits within 10-30 miles distance from their home to the patient’s place of residence, in order to provide skilled care services as ordered by referring physician.
Summary
The Licensed Vocational Field Nurse is responsible for providing direct patient care under the supervision of a registered nurse. responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Duties, Responsibilities, And Accountabilities
Responsibilities include but are not limited to assisting with the following:
• The Licensed Vocational Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse
• Provide skilled nursing services/interventions in accordance with physician orders
• Assesses patient needs, plans, evaluates, and modifies care to meet goals of skilled nursing interventions
• Collaborates with all departments to plan and evaluate team goals for each patient
Per-Diem Status
(Skilled Nurses can choose areas of their choice and choose own flexible schedule). MUST be able to cover any or all of the following areas:
• Rancho Cucamonga, Fontana, Rialto, Ontario, San Bernardino (Post #1)
• Moreno Valley, Perris, Menifee, Sun City, Homeland ( Post #2)
• Lake Elsinore, Canyon Lake, Wildomar, Temecula, Murrieta ( Post # 3)
• Redlands, Highland, Calimesa, Yucaipa, Banning, Beaumont, Cherry Valley, San Bernardino, Colton, Loma Linda ( Post #4)
Minimum Requirements include:
• Graduate of a National League for Nursing accredited school of nursing
• Must have minimum 1 year of experience in nursing as an LVN
• Able to see patients at least 5-8 visits per week
• Experience in EMR software (preferred)
• Experience with Wound Care
Required Licenses and Certifications:
• LVN License in California
• Valid Driver’s License & Auto Insurance
• Current BLS/CPR certification
• Physical and TB from within the last 6 months
