INTRACARE COMMUNITY OUTREACH COORDINATOR -Carrollton, TX

Responsibilities

• Generates new patient opportunities by nurturing, building and cultivating relationships with seniors and stakeholder partners during community events
• Describe IntraCare’s value propositions within the senior centers, living communities, social group locations & face to face opportunities
• Educates prospective patients on our value proposition and healthcare model to drive new member growth.
• Works with insurance agents during community events with the goal of capturing new patient assignment
• Follows up with prospective patients to facilitate first visit appointments
• Maintains relationships with enrolled patients to positively impact retention.
• Utilizes CRM or other tracking tools to create and document interactions with prospect patients and ensures effective management of new patient acquisition efforts.
• Maintains a sufficient par level of IntraCare marketing collateral & branded promotional items for all community events
• Communicate with other IntraCare departments on community event outcomes, new opportunities, & challenges.
