    Job Title: Data Coordinator - Sustainable Outreach & Navigation (SON)
    Position Reports To: Program Manager
    Hours: Full-time, occasional evening and weekend work required
    Hourly Wage: $3,466.67 - $3,956.67 per month

    Job Summary: The Data Coordinator - SON will the team of Community Health Workers (CHWs), Supervisor and Program Manager within the AVPH’s CHW SON program. This role includes both performing direct outreach and system navigation tasks (20% of the time) with the CHWs and providing administrative support (80% of the time) to ensure effective data entry and monitoring. The Data Coordinator will assist Supervisor/ Program Manager in monitoring daily referral/outreach tracking to ensure appropriate referral assistance is provided.

    Responsibilities and Duties:

    • Responsible for overseeing data collection, tracking and management of assessments, monitoring and reporting of outcomes, and troubleshoots data issues.
    • Monitor the referral process and work with Supervisor to ensure that CHWs utilize the Resource Dashboard effectively, following up on service linkages to close referral loops.
    • Monitor outreach and referral data entry into the System Navigation Portal, ensuring accuracy and timeliness.
    • Assist in preparing monthly and quarterly progress reports, highlighting outreach metrics, success stories, and any program adjustments needed.
    • Attending monthly data meetings with DPH to discuss project updates and reports issues with data entry platforms.
    • Stay abreast of current and emerging health issues to assist with educating teams and informing the local community response.
    • Oversees and conducts appropriate quality checks to ensure accuracy and quality of data.
    • Process a minimum of 5 intentional engagement activities per month from outreach types, which may include door-to-door engagement, community canvassing, presentations, tabling at events, and individual interactions. This target ensures consistent community presence and engagement aligned with program goals.
      • Provide at least 5 resource referrals and 5 linkage referrals
    • Engaging with community members to provide health education and connect them to available LA County services.
    • Offering direct referrals and connections to services for individuals impacted by COVID-19 and other public health issues.
    • Assisting community members in navigating complex health and social service systems and following up on referrals to ensure successful linkage to services.
    • Providing culturally and linguistically appropriate information on public health priorities, including health education, protective equipment distribution, and COVID-19 vaccine resources.

    Experience & Requirements:

    • Bachelor's degree in Public Health, Epidemiology, Public Health Statistics, or Communicable Disease Control, or other relevant degree.
    • Minimum of one (1) year of experience with data entry and/or management OR a minimum of five (2) years of experience managing data or evaluation of grant-funded public health, social service and/or similar programs.
    • Experience working with culturally diverse and underserved populations, with a strong commitment to equitable service provision.
    • Passion for community health and wellness, with a specific interest in the focus areas of health and public health principles.
    • Ability to work independently and as part of a team, demonstrating empathy, cultural competency, and commitment to improving community health.
    • Proficient in Microsoft Office, specifically Excel and comfortable with data management tools like Power Bi.
    • Bilingual or multilingual skills preferred but not required.
    • Current and maintained CA driver’s license.
    • Current/maintained auto insurance.
    • Own transportation for work-related purposes.
    • Must pass background check and TB test clearance.
    • COVID-19 vaccinated card recommended.

    Physical Requirements and Working Conditions:

    • Primarily an in the field and office environment; with conditional hybrid – remote option.
    • Will be required to sit, perform repetitive physical and mental activities, have face-to-face and telephone contact with individuals, work with others in a group or team, and work with external customers or the public.
    • Office, and possible community settings.
    • Work is often performed under stressful and emotional conditions.
    • This position requires lifting/carrying up to 25lbs., stooping, bending, squatting, walking, running (may be needed).
