Job Summary
|Reporting to the Program Director, the Day Center Manager is a member of the High Desert PACE management team, also spearheading engagement and personal care services. As a crucial member of our interdisciplinary team, you will be responsible for overseeing the operations of our day center, ensuring the highest quality of care and services for our elderly participants. The PACE Day Center Manager plays a vital role in creating a supportive and enriching environment that promotes the health, well-being, and independence of our participants.
Responsibilities
Program Management: Guide effective planning, implementation, evaluation, and improvement of operational processes. · Oversee the engagement department assessments and interventions to assure regulatory compliance and timely documentation. ·Develop and implement engaging and therapeutic activities tailored to the diverse needs and interests of participants. ·Participating member of IDT, facilitates and provides effective leadership for care plan meetings. ·Collaborate with the interdisciplinary team to integrate medical, social, and recreational services into the daily programming.
Staff Supervision, Development, and leadership: Hire and direct key staff including the personal care attendants, dietitians, and engagement/activities team members. ·Maintain and supervises all services of the day program including recreational activities, dietary services, and personal care activities including but not limited to medical escort schedules, meal service, and social activities led by personal care attendants. · Provide ongoing support, guidance, and professional development opportunities to staff to ensure the delivery of high-quality care and services. ·Foster a positive and inclusive work environment that promotes teamwork, communication, and mutual respect. ·Collaborate with Program Director to plan and facilitate effective interdisciplinary team (IDT) and other participant-care related meetings, ensuring they are organized and engaging, and that follow-up items are documented and tracked. ·Collaboration with the Program Director, Monitor center and department budgets, performance to utilization targets, productivity, and specific program objectives. ·Collaboration with the Program Director, oversight of day center from a maintenance/facilities/safety perspective. ·Collaboration with the Program Director, involved in implementations of Quality Improvement Activities · Other duties as assigned.
Participant Care and Support: Ensure that participants are oriented to the program and have opportunities to express their opinions about the care and services they receive. ·Monitor participants' progress, address any concerns or changes in condition, and coordinate additional services as needed.
Compliance and Documentation: Assist in development of Policies and Procedures ·Ensure compliance with all applicable regulations, policies, and procedures governing PACE Day Center operations, including safety standards and licensing requirements. ·Maintain accurate and up-to-date documentation of participant records, care plans, and program activities. ·Prepare reports, participate in audits, and collaborate with regulatory agencies as necessary.
Community Engagement and Outreach: Build and maintain positive relationships with participants, families, caregivers, and community partners. •Serve as a liaison between the PACE program and external stakeholders, advocating for the needs an interest of participants.
Education & Training • Bachelor's degree in social work, recreational therapy, healthcare administration, or a related field (Master's degree preferred). • Minimum of 3-5 years of experience in a leadership role within a healthcare or senior care setting. • Strong understanding of aging-related issues, dementia care, and person-centered care principles. • Excellent organizational, interpersonal, and communication skills. • Ability to multitask, prioritize, and problem-solve in a fast-paced environment. • Proficiency in Microsoft Office Suite and electronic health record systems. • CPR/First Aid certification and any other relevant certifications/licenses required by state regulations.

Ability to integrate medical, social, and recreational services to promote holistic care and well-being. ·Assessment and Care Planning: Proficient in conducting comprehensive assessments of participants' physical, cognitive, functional, and psychosocial status to develop individualized care plans. Ability to prioritize and coordinate services, monitor progress, and modify care plans as needed in response to changes in condition or goals. ·Regulatory Compliance: Thorough understanding of relevant federal, state, and local regulations, licensure requirements, and accreditation standards governing PACE day center operations. Ability to ensure compliance with documentation, safety, infection control, and quality assurance protocols. ·Community Engagement: Strong interpersonal and networking skills to build and maintain positive relationships with participants, families, caregivers, and community partners. Ability to represent the PACE program effectively, advocate for participant needs, and promote awareness of services within the community. ·Problem-Solving and Decision-Making: Sound judgment and critical thinking skills to identify issues, analyze complex situations, and develop practical solutions. Ability to make informed decisions under pressure, prioritize tasks, and manage competing demands effectively. ·Technology Proficiency: Comfortable using technology and electronic health record systems to maintain accurate participant records, generate reports, and communicate with team members. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and other relevant software applications. ·Compassion and Empathy: Genuine empathy and compassion for the aging population and their families, with a commitment to providing person-centered care that preserves dignity, autonomy, and quality of life. Ability to establish rapport, build trust, and respond sensitively to the emotional and cultural needs of participants. ·Professional Development: Commitment to ongoing professional growth and development through participation in training, continuing education, and professional organizations related to gerontology, healthcare administration, or recreational therapy. Willingness to stay abreast of best practices, emerging trends, and innovations in the field of aging services.
