POSITION: Day Support Case Manager PROGRAM: Day Program - Developmental Disabilities Division STATUS: Full-time, Non-exempt

JOB SUMMARY Assist the Day Support Coordinator in the daily operations of the Day Support Program. Ensure that all client paperwork is completed and up to date. Ensure that client needs are being met and that there is open communication among staff, client families, and community resources. Promote team cohesiveness within the center.
ORGANIZATIONAL RELATIONSHIP This position is supervised by the Director of Day Support Services and will provide overall supervision of the center and staff in the absence of the Day Support Coordinator.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Promote a positive work environment.
Provide leadership and guidance to mentors in the daily operations of the Day Support Program.
Exhibit professionalism with staff and promote a professional work environment.
Follow and abide by all personnel and program policies and procedures.
Attend regular supervision with the Director.
Keep the Director informed of any issues occurring in the day support.
Train new staff on required client paperwork and complete clients’ daily training objectives.
Attend regular staff meetings and in-service training programs.
Complete all required yearly training by December of each year.
Conduct and document monthly Day Support Program smoke alarm and fire drill tests.
Screen referrals and coordinate admissions.
Consult with referring agencies, care providers, and residential social workers regarding client progress, needs, and service coordination.
Develop service plans for each client using information from client observations, client and team input, and by completing assessment materials. Ensure service plans are completed on time and are an accurate representation of the clients’ learning and assistance needs.
Initiate and coordinate client meetings as needed.
As scheduled, report on client progress, needs, and behaviors at Annual and Quarterly meetings.
Intervene and provide guidance to mentors as client emergencies arise.
Disperse caseloads each morning.
Provide mentoring services (training and personal care) with clients as needed.
Meet all paperwork reporting and deadline requirements.
Complete: Release of Information forms, Human Rights Forms, Emergency Medical Forms
Complete: Information Forms when changes occur in the client’s status.
Complete discharge summaries as required. Forward charts to be closed to the Quality Improvement within 30 days of discharge.
Calculate monthly client billing and forward the completed Certification of Billable Hours Form to the Timberlake Office Manager within two days following the end of the month.
Review mentor progress notes, observe client/staff interactions, and instruct mentors on effectively teaching their clients skills.
Complete Annual Reviews, Quarterly Reviews, and progress notes and data sheets after assessing the client’s status and forward them to the referring agency.
Audit client charts periodically for completeness and ensures required annual forms are updated and filed.
File and mail quarterly reports as required.
Assist the Finance Department in tracking and resolving billing problems.
Perform other duties as may be assigned.
ESSENTIAL KNOWLEDGE, SKILLS, AND ABILITIES Demonstrated organizational skills and the ability to multitask. The ability to be proficient orally and in writing and have efficient computer skills is required. Demonstrated ability to prioritize and meet paperwork deadlines. Must be able to assess participants’ strengths and needs. Leadership skills and the ability to provide oversight of the Day Support Program and staff when the Coordinator is unavailable.
TRAINING, EDUCATION, AND EXPERIENCE A minimum of a Bachelor’s degree in a human services field or an equivalent combination of training/education and/or experience is required. A human services field may include social work, psychology, sociology, counseling, psychiatric or vocational rehabilitation, special education, and gerontology. Additional degrees related to a human services field may be considered after a review of the college transcript. A minimum of five years of experience in the field of Developmental Disabilities is required.
SPECIAL REQUIREMENTS The applicant must submit a current driving record (7 years) and possess a driving record that complies with DePaul policy before hiring. Must have and be able to use personal transportation. Must be willing to have a criminal and child protective services (CPS) background report conducted upon hire. The applicant shall not have been convicted of any barrier crime(s) on the criminal background report. The applicant will have no founded disposition of child abuse or neglect as documented by the CPS background report. A non-negative result of the pre-employment drug screen will terminate the application process or result in termination. Documentation of a PPD test or chest x-ray indicating the absence of TB must also be submitted. The applicant must be able to obtain certification in First Aid, CPR, and crisis management of challenging behaviors and administration of medication; and able to lift a minimum of 50 pounds.
WORK ENVIRONMENT Most of the work will be completed at the designated Day Support Center. Meetings with community resources and others and mentoring in the community will require travel by car, but rarely if ever, overnight.
