    Job Summary: The Nurse Supervisor plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The nurse supervisor builds from the resources of the community to plan and direct services to meet the needs of the client and families within their homes and communities. Participates in on-call duties as requested.

    Qualifications:

    Registered Nurse (RN) with a license in good standing in Maryland

    Current CPR certification

    Minimum of two years of nursing experience as an RN

    One year of home care experience

    One year of Supervisory experience preferred

    Works collaboratively with clients, families and staff

    Ability to meet bonding criteria

    Valid driver’s license, available personal vehicle

    Responsibilities/essential functions:

    Complies with HIPAA, federal, state and local regulations and safeguards all client related

    Information

    Ability Assists with implementation, adherence and monitoring of compliance with HIPAA,federal, state and local regulations to ensure the agency is compliance with regulatory

    standards

    Be readily available either in person, by telephone or by other means to the staff at times when services are being provided

    Maintains and adheres to agency policies and procedures to ensure care delivery meets thneeds of the clients and follows professional practice standards

    Immediately reports any problems with client and/or staff to the Administrator and assist with the investigation and outcomes of concerns, as needed

    Assists the Administrator with quarterly clinical chart review to meet regulatory standards

    Perform supervisory visits and monitor outcomes of delegation to ensure all direct care staff are adhering to the plan of care to meet the needs of the client

    Conducts and documents competency and training of direct care staff to ensure safe quality care is provided to clients and adheres to agency policies and procedures

    Provides guidance and support to direct care staff

    Instructs HHA’s and CNA’s and other appropriate staff on proper techniques for using equipment and procedures

    Conducts direct care staff performance evaluations and assists with the development of revised/new job standards as needed

    Assists as requested with recruiting, hiring, evaluating and terminating direct care staff personnel

    Performs the initial client assessment and reassessment to determine the needs of the client

    Educates, coaches and monitors client and direct care staff on Plan of Care and changes to the plan to ensure client’s needs are met

    Protect clients, families and other agency personnel by adhering to infection control policies and procedures to ensure safe quality care is provided to the client

    Identifies discharge-planning needs as part of the Plan of Care development and implements prior to discharge of the client

    Demonstrates effective time management skills with daily assignments

    Maintains clean and neat work environment

    Demonstrates a positive and professional manner according to agency policy

    Accepts assignments willingly and perform other duties as assigned

    Maintains acceptable attendance status, per agency policy

    Attends mandatory staff meetings, in-service programs, continuing education, seminars, and self study programs as required

    Physical Elements

    Sufficient clarity of speech and hearing or other communication capabilities, with or without reasonable accommodation, to enable the employee to communicate effectively;

    What you’ll be doing:

    Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.

    Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings.

    Providing interventions with a focus on achieving realistic client outcomes.

    Reporting changes to update the plan of care to reflect progress towards goals and outcomes.

    Maintaining client records showing systematic assessments, planning interventions, and evaluations.

    Initiating referrals as needed to other health team members.

    What we’re looking for:

    Graduate of an accredited school of professional nursing.

    Current license as a Registered Nurse (RN).

    Current CPR certification.

    Access to reliable transportation.

    Maintains confidentiality in all aspects of the job.

    Knowledge of safety and infection control practices.
