    Facility Overview

    Our client is a well-established Life Plan Community providing high-quality assisted living and memory support services. The interdisciplinary team is dedicated to delivering compassionate and comprehensive care, ensuring the best outcomes for residents.     Position Summary & Responsibilities

    The Delegating Nurse will function as the RN Delegating Nurse for Assisted Living and Memory Support. The role also supports various nursing initiatives within the Skilled Nursing neighborhood, collaborating with leadership to enhance clinical systems, provide staff education, and manage infection control/prevention activities.

    Key Responsibilities:

    • Conduct daily rounds to monitor safety, sanitation, and the overall environment in Assisted Living (AL) and Memory Support Assisted Living (MSAL).
    • Engage with residents and families to ensure physical and mental well-being needs are met.
    • Lead daily staff stand-up meetings to review admissions, orders, and resident concerns.
    • Oversee nursing functions to maintain efficient operations and quality care.
    • Attend administrative meetings and coordinate room turnover with families and facility staff.
    • Manage and maintain accurate resident charts, including Uniform Assessment Instrument (UAI) and mental health screenings.
    • Ensure proper documentation and compliance with regulatory standards.
    • Supervise the Wander Guard program for residents at risk of elopement.
    • Assist with infection control procedures, including PPD screening and renewals.
    • Coordinate hospice referrals and transition planning when necessary.
    • Facilitate resident and family care plan meetings to review care levels and address concerns.
    • Work with Admissions and clinic staff to ensure smooth transitions for new residents.
    • Maintain compliance with Department of Health and Department of Social Services regulations.

    Admissions & Assessment

    • Conduct assessments for resident transitions between care levels and provide guidance
    • Collaborate with the clinic and Admissions team to coordinate move-in procedures.
    • Complete required documentation for new residents, including mental health screenings and UAIs.
    • Ensure all resident charts are updated with accurate physical and mental health records.

    Supervisory Responsibilities

    • Lead/manage AL and MSAL staff, ensuring productivity, efficiency, and quality of care.
    • Conduct staff training and cross-training as necessary.
    • Address performance issues and implement progressive discipline when needed.
    • Complete annual performance evaluations and update Assisted Living procedures as necessary.

    Professional Development & Compliance

    • Maintain all necessary nursing certifications and licensure.
    • Stay up-to-date on state and federal healthcare regulations.
    • Participate in facility projects, committees, and quality improvement initiatives.
    • Ensure compliance with all safety and emergency procedures.
    • Maintain CPR certification and oversee on-site training programs.

    Requirements

    • Bachelor’s degree in Nursing (BSN) preferred; Associate’s degree with equivalent experience accepted.
    • Registered Nurse (RN) license required.
    • Five to seven years of nursing experience, preferably in Assisted Living, Memory Support, or Long-Term Care.
    • Strong knowledge of electronic medical records (EMR) and general office applications.
    • Experience in supervisory roles with a proven ability to lead teams effectively.
    • Current BLS Certification.
