DESCRIPTION:
The Designated Coordinator provides supervision and support to direct support staff and is responsible for the evaluation of activities for individuals receiving services. The Designated Coordinator complies with the Individual Support Plans for individuals receiving services according to federal and state regulations and the organization’s policies and procedures to ensure the quality delivery of services to all individuals served.
JOB RESPONSIBILITIES:
The Designated Coordinator job responsibilities include:
Train, schedule, and provide ongoing support and evaluation of direct care staff.
DHS 245D compliance and paperwork.
Manage services (or 24-hour supports) emergency needs.
Evaluate of the effectiveness of service delivery, methodologies, and progress on the person's outcomes
Communicate with individuals, guardians, case managers, behavior analysts, and other members of the expanded support team to decide on services needed to support each individual most effectively.
Maintain and summarize data for reports.
Coordinate appointments for individuals, as requested.
Problem solver when emergencies arise.
Works well under pressure.
Work direct care (as needed).
Will be required to be in an on-call rotation.
General Accountabilities
Evaluation of the effectiveness of service delivery, methodologies, and progress on each individual’s outcomes based on measurable and observable criteria for identifying when the desired outcome has been achieved as identified in their CSSP/SP-A.
Ensuring that services are provided according to the individual's Community Service Support Plan (CSSP) and Support Plan Addendum (SP-A).
Ensure that all information and documentation related to service provision is being maintained accurately and consistent with licensing requirements and company policy and standards.
Maintain effective and timely communication and teamwork with all stakeholders.
Manage a staffing schedule that meets the needs of the program/individuals supported as identified in their SP-A and service agreements within the budget.
Provide oversight, supervision, and leadership to the Direct Support Professionals which includes oversight of their scheduling, training, and overall performance.

APPLICANT REQUIREMENTS: To qualify for the position, applicants must:
Have a current Driver’s License (Required)
Pass a Department of Human Services background check (Required)
Work independently and have strong organizational skills
Demonstrate strong written and oral communications skills
Proficiently use Microsoft Office, Teams and similar platforms
Have experience and understanding of 245D compliance
EXPERIENCE/EDUCATION:

Qualified candidates must have one of the following:
2 years’ experience working with individuals that have high behavioral support needs, preferred.
Must be willing to commute to various Resolute Care sites/programs.
Minimum of 1 year Supervisory experience, preferred
Bachelor’s degree in a field related to human services, preferred (e.g. psychology, social services)
Bachelor’s degree in a field related to human services, and 1 year of full-time work experience providing direct care services or
Associate degree in a field related to human services, and 2 years of full-time work experience providing direct care services or
Diploma in a field related to human services from an accredited postsecondary institution and 3 years of full-time work experience providing direct care services or
Minimum of 50 hours of education and training related to human services and disabilities; and 4 years of full-time work experience providing direct care services
