    Job Summary

    We are expanding our commitment to holistic and preventative care by integrating nutrition services directly into our clinical workflow. We're seeking a dynamic Registered Dietitian or highly credentialed Nutritionist to join our team — someone with both clinical expertise and an entrepreneurial mindset.

    This is a unique opportunity to embed yourself into a thriving primary care practice and shape how nutrition services are delivered across our patient population. You'll collaborate directly with our physicians and chronic care team, manage your own patient panel, and have the freedom to innovate, grow, and potentially establish a long-term partnership.

    Responsibilities

    Provide individualized Medical Nutrition Therapy (MNT) and counseling for patients with chronic conditions (e.g., diabetes, hypertension, obesity, hyperlipidemia, CKD).

    Develop and lead group nutrition education sessions and classes.

    Work closely with physicians to build efficient referral workflows within the EHR.

    Track and document encounters for billing and quality reporting.

    Educate staff on the role of nutrition in chronic care management.

    Contribute to patient engagement efforts, health fairs, or outreach initiatives.

    Qualifications

    Registered Dietitian (RD/RDN) credential required – or advanced nutritionist certification (CNS) with strong clinical background.

    Strong understanding of insurance billing (CPT 97802/97803/97804, G0270/71) and ICD-10 codes supporting MNT.

    Entrepreneurial drive and a collaborative approach to building services within a practice setting.

    Excellent communication and interpersonal skills

    The ability to compassionately counsel people making major life changes

    The ability to work independently and within a team environment

    Strong problem-solving and analytical skills

    Knowledge of value-based care models (CCM, RPM, APCM) is a plus.
