Maintain records of patient care, condition, progress, or problems to report and discuss observations with the RN or case manager.
*Interact, support, and converse with clients to keep them mentally healthy and alert.
*Provide clients and families with emotional support and instruction in areas such as living independently, or adapting to disability.
*Perform a variety of duties and needs as requested by clients, such as obtaining household supplies or running errands.
*Accompany clients to doctors' offices or on other trips outside the home, providing transportation, assistance, and companionship.
*Services will be provided to HCS and IDD clients and their families.

We provide services that assist an individual in gaining access to needed medical, educational, and/or other services to locate, coordinate, and monitor necessary appropriate services. They may also encourage the use of cost-effective medical care by referring to appropriate providers and discouraging the overutilization of costly services. Case management may also provide necessary coordination of non-medical services such as vocational rehabilitation, education, and employment when the services provided enable the individual to function at the highest level.
