Job Summary
We are looking for a Home Care Aide to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
Responsibilities
Personal Care:
Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, medication administration, and toileting.
Support and Supervision:
Provide companionship and emotional support to clients.
Support clients in developing and maintaining social relationships.
Supervise and assist with recreational activities and community outings.
Household Management:
Assist with light housekeeping tasks, such as laundry, meal preparation, and cleaning.
Ensure a safe and clean living environment for clients.
Documentation:
Maintain accurate and timely documentation of services provided.
Complete daily logs, incident reports, and other required documentation.
Compliance and Training:
Follow company policies, procedures, and protocols.
Participate in ongoing training and professional development.
Qualifications
High School Diploma or GED
One year prior professional experience preferred
Driver’s license preferred
CPR/First Aid certification preferred
