Job Summary
Main Objective: Provide person-centered direct support services to individuals with disabilities in their home and community
Major Duties:
General Accountabilities
Job Responsibilities:
Provide direct care to assigned individuals
Perform any necessary self-care tasks, including but not limited to basic personal hygiene, grooming, feeding, ambulation tasks, medical monitoring, and monitoring for health and safety.
Ensure the individual’s environment is safe; assist with maintaining it, if necessary
Remind and/or assist individuals with self-administration of medications (after Core A/B training is successfully completed)
Provide support for community activities and engagement
Assist with and track all necessary PCISP and PBSP goals and strategies
Encourage individuals for movement toward his/her wants and needs
Assist individuals to plan meals, shop for groceries and other items, prepare/serve meals, feed
Help individuals with household cleaning and simple maintenance tasks
Assist with toileting, such could include reminders, verbal prompting, and/or physical assistance
Complete documentation for all delivered services within the identified timeframe
Report all incidents immediately with an internal report completed within 12 hours of incident
REPORT ALLEGED, SUSPECTED OR ACTUAL ABUSE, NEGLECT, OR EXPLOITATION IMMEDIATELY
Attend interdisciplinary team meetings as requested
Other duties as assigned
Job Qualifications
Eduction: Minimum of a high school diploma, GED, or equivalent
Experience: One year of paid or volunteer experience preferred
Requirements: Must be 18 years old, possess a valid and unrestricted driver's license, and have a clean criminal history
