Job Summary

Exceptional Tours is a Home and Community Based Services Agency supporting people with intellectual/developmental disabilities. We are looking for Direct Support Professionals located in the Southeast Region of Pennsylvania, to join our team! You will directly work with participants following a one-on-one care plan providing Community Participation Support, In Home and Community Support or Respite. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets participant needs. Strong verbal and written communication skills are needed to succeed in this role. Prior experience working with people with intellectual/developmental disabilities and or autism is a plus.

Responsibilities

Provide Community Participation Support, Companion, In- Home and Community Support or Respite Services to assigned client as defined by ODP's service definitions.

Maintain current driving/vehicle credentials and proper insurance as required by ODP

Plans daily activities using the ISP, as a guide.

Supervises clients in the community to ensure they remain healthy and safe

Document Daily Visit notes in Carsmartz360

Obtain and provide documentation of necessary training required by ODP to remain compliant

Maintain contact with Client’s primary care provider to provide services as needed.

Report Incidents to Client Care or other designated Admin team member

Qualifications

High School Diploma or GED

One year prior professional experience

Current Driver’s license and insured vehicle required

Community Participation Support Certification

Modified Medication training Certificate
