Position Title: Director Case Management

The Role you will play as the Director of Case Management
As the Director of Case Management you would have the ability to create/design this program from scratch, to truly make it your own, to leave your personal stamp on the organization for years to come!
Working closely with Crystal Frantz, Executive Director of Care Management, and as part of the leadership team you will have your finger on the pulse of the organization you will be supported to design and implement relevant programs.
A successful candidate will have acute care CM experience, as a supervisor or team lead, manager, or director. SRMC is a smaller facility and a great community, very safe, has the newest stores and is close to Alb. It is about 30 – 40 into Santa Fe, good team, low turnover and has only a couple of travelers.
This is an opportunity to be part of a closely knit leadership team who enjoys having fun together but also has a good culture of accountability.
The CM team is creative and seasoned, strong UR/CM practiced and are high level, well trained folks. They like some flexibility from their leadership and are described as pretty self-sufficient.
They have around 7 Case managers, 5 Utilization Review, 2 CM assistants, there is no supervisor between you and them at this time, however as you and the organization continue to grow the department it would be a logical layer to introduce.
Crystal expects someone to have come from acute care setting with management experience. Somebody who would align themselves with their values and be a great communicator. They have roughly 60 beds, Center of Excellence, and sometimes it’s a roll up sleaves if you have call outs and get on the floor doing the work. They are a level 3 trauma.

Crystal has been with UNM for 28 years and in her current role for 15. She started as a care coordinator 29 years ago and like the teaching environment, enjoys working in a large academic medical center and wanted to be were cutting edge innovation is done. After getting her masters she started managing clinics and then was asked to come back into Care Management as the Executive Director. Lomas campus is large at 500+ beds, and Sandoval is 70 beds that are typically pretty full. Some of the initiatives are centered around improving throughput and length of stay.

Rio Rancho is the community most who work at Sandoval live in, low crime rate, good economic activity, and is progressive in offering environmental friendly services such as their Spring Green – they pick up residents spring clippings/yard waste for free for example. *New Mexico reports all incidents as crime, meaning if someone where to report a planter stolen from their front porch it shows in their statistics, most cities only report violent or certain types of crimes – just a note for those who are not familiar with Alb, NM – the stats are misleading.
