    Santa Clarita Valley, California

    The Director of Case Management contributes to the development and improvement of clinical care, enhancing cost effectiveness while providing quality care. The Director of Case Management establishes outstanding rapport and coordinates with physicians, clinicians, health plans, and local providers. The Director of Care Management oversees the Care Management and Social Services departments. The Director establishes best practice programs to ensure effective and efficient coordination of patient care in accordance with clinical, organizational, and other applicable guidelines and standards. The Director assists to balance clinical and financial concerns with a family’s needs and the patient’s quality of life.

    Qualifications:

    • Registered Nurse required
    • Bachelor’s degree in nursing required
    • Master’s degree preferred
    • Director of Case Management experience in an Acute Hospital setting
    • Progressive experience in a management or administrative capacity in hospital Case/Care Management
    • Specialty certification in Case Management and/or Administration is preferred

    Responsibilities:

    • Identifies and implements innovative approaches to ensure positive clinical and financial outcomes related to the appropriate application and allocation of healthcare processes and resources for the patient population
    • Responsible for the overall planning, direction and implementation of roles, processes and programs for the Care Management and Social Work
    • Leads system capacity management processes to actively manage LOS and advance the progression of care of patients with significant barriers to discharge
    • Leads system extended length of stay reduction initiatives
    • Leads initiatives and projects related to patient centered care and clinical resource utilization as needed
    • Advises Senior Management on issues and policies impacted by regulatory changes
    • Responsible for the development, organization and management of operations of the Care Coordination/Social Work department to ensure medically appropriate and cost-effective care
    • Defines roles and responsibilities for Care Management and develops procedures to ensure seamless continuity for patient Care Coordination and works with clinical departments to integrate the Care Management model as a component of patient centered care delivery
    • Develops and implements processes to ensure appropriate Care Coordination and transitions planning for all patients with the community and in conjunction with other entities
    • Collaborates with all clinical departments to develop and implement service oriented and “systems” approaches for Care Coordination
    • Plans for and responds to regulatory changes and audit activities to reduce financial risk and improve revenue recovery
    • Collaborates with system financial leaders on payor contract reviews to optimize revenue
