Director of Case Management

About the Job: The Director of Case Management will develop and implement case management strategies, oversee a team of care coordinators, and ensure staff are trained in value-based care (VBC). Hands-on leadership and skilled nursing facility (SNF) experience are essential.

What You’ll Do:

• Lead case management strategy and team development.
• Train and manage 15-20 case managers and care coordinators.
• Oversee transitions of care and patient-family engagement.
• Collaborate with clinical leadership for VBC improvements.

What You’ll Bring:

• Licensed RN (LVN considered for stellar candidates with VBC and SNF experience).
• Strong managerial experience with direct reports.
• Expertise in VBC and case management.
• Excellent communication and problem-solving skills.

Location: Hybrid role, with at least two days per week in the Costa Mesa office.

Compensation & Benefits:

• Base salary: $120,000 - $150,000 (potential for higher with experience).
• Annual incentive plan (AIP) up to 10%.
• Equity eligibility after one year.
