Touching Hearts at Home of Southwest Boston is hiring a Director of Client Care to oversee and enhance the quality of care we provide to seniors in their homes.

    What You’ll Do As Director of Client Care: As the Director of Client Care, you’ll lead the charge in ensuring our clients receive outstanding care. From coordinating caregivers to communicating with families, you’ll be at the heart of our mission to improve the lives of seniors.

    Key Responsibilities:

    Act as the primary contact for caregivers, clients’ families, and senior care professionals regarding care-related concerns.

    Ensure care quality through routine home visits and regular goodwill calls.

    Build strong relationships with referral sources and families, providing expert advice for client well-being.

    Supervise and mentor caregivers through coaching and hands-on training to improve client outcomes.

    Draft and update care plans to reflect the most current client needs.

    Occasionally fill caregiver shifts or provide on-call support as needed.

    Represent our organization with professionalism and empathy, fostering trust within the community.

    Who We’re Looking For: We’re searching for a dedicated professional who thrives in a collaborative environment and is passionate about senior care.

    Required Qualifications:

    3+ years of social work or nursing experience in senior care (home care or community-based setting preferred in roles like Home Care Case Manager, Elder Care Coordinator, RN/LPN, Resident Care Director, Health & Wellness Director, etc.).

    Comprehensive understanding of local senior care resources and services.

    Passion for helping elders remain safe and independent wherever they call home.

    A proactive, action-oriented mindset and a “can-do” attitude.

    Strong organizational, verbal, and written communication skills.

    Willingness to step into various roles and fill shifts as needed.

    A valid driver’s license and reliable, insured vehicle.

    Work Schedule:

    Full-Time, Hybrid Role: Monday – Friday, 8:00 AM – 4:30 PM. Occasional pre-office hours, post-office hours or weekend needs will arise.

    On-the-Go: Regular travel to client homes or senior communities within the Southwest Boston area.
