POSITION PURPOSE / SUMMARY
The primary responsibility of this position is to transport Elkhart County residents to and from medical, grocery, banking pharmaceutical and other personal business appointments.
PREREQUISITES
· Must be a minimum of 21 years of age
· Pass pre-employment and random drug screening
· Pass criminal background check
· Tuberculosis screening
· For-hire driver’s license endorsement
· Active vehicle insurance and qualify for placement on corporate insurance
· CPR and First Aid certification (provided by company)
· Passenger Assistance Training certification (provided by company)
· Ability to learn and use an electronic tablet and GPS
DUTIES AND RESPONSIBILITIES
· Transport client(s) to and from medical, grocery, banking, pharmaceutical and other personal business appointments throughout all areas of Elkhart County and surrounding areas · Utilize all safety precautions implemented by the company to protect every client and staff person transported, including but not limited to disease prevention and securement devices · Complete daily documentation of every transport provided · Complete daily maintenance documentation for monitoring of required service by the Transportation Manager · Participate in and successfully complete annual training, including RTAP and CPR/First Aid certification, as directed by the Transportation Manager · Maintain a For-Hire Endorsement, active driver’s license, clean motor vehicle record and follow all state laws · Maintain personal vehicle insurance, providing documented proof to COA · Perform in a professional manner and in accordance with corporate policy when interacting with clients and co-workers · Understand and conduct self in accordance with corporate employee handbook · Develop collaborative environment with team co-workers · Maintain confidentiality of corporate and client information · Additional tasks as assigned by supervisor
PHYSICAL DEMANDS · This position requires good physical and mental agility and alertness · Must have the ability to perform physical tasks in accordance with planned activities · Extensive walking and/or standing may be required at times · Daily tasks involve lifting up to 25lbs, carrying, bending, reaching, sitting, and standing · Adequate vision and hearing abilities are required for this position · Ability to maneuver (lift, turn, push and pull) a wheelchaired individual up and down a ramp and forward-facing for a distance
· Ability to physically bend to secure wheelchairs
