Job Summary:
Provides personal care and related services in the home. Functions under direct instruction and supervision of an RN or therapist.
Essential Functions:
Follows written instructions for patient care as prepared by an RN or Therapist.
Assists patients with personal care (which includes lifting and transferring patients without assistance) to preserve good personal hygiene while maintaining a healthful, safe environment.
Performs simple procedures as an extension of therapy services, such as ambulation, ROM, and exercise.
Assists in administering medications that are ordinarily self-administered; prescribed under written direction of Physician or as directed by a nurse, as permitted by State regulations.
Reports changes in patient’s condition and needs to Supervisor or RN.
Maintains record of services performed and of apparent condition of patient.
Establishes and maintains a good working relationship with the patient/family and co-workers of the Agency.
Participates in patient care conferences to provide input requiring continuum of patient care.
Performs household services essential to health care at home.
Additional Responsibilities: List infrequently performed and non-essential tasks.
Performs other related duties as assigned or requested.
Adheres to Texas Association for Home Care Code of Ethics.
Conforms to all applicable Agency policies and procedures.
Participates actively in continuing education and in-services.
Maintains confidentiality of patient information and business trade practices.
Assumes accountability for reporting incidents and complaints according to Agency policy.

Minimum Position Qualifications:
•Education: High school diploma or GED
•Experience: NA
•Training: Successful completion of training and competency evaluation program or competency evaluation program or nurse aide training and competency evaluation program approved by the state without a contiguous 24 months lapse
•License / Certification: Driver’s license and proof of current auto liability insurance; No Listing in the OIG Excluded Provider Listing; listed in good standing on state nurse aide registry or requirements of a state licensure program meeting federal regulations
Preferred Position Qualifications:
Experience: 1 year nurse aide or home care aide
License/ Certification: current CPR
Environmental Conditions: Moderate noise level; Category A BBP risk; moderate stress and emotional demands.
