The Company AmCare Medical Group (AmCare) is multi-provider primary care medical practice on a high growth trajectory. We are seeking multiple experienced and certified Nurse Practitioner for primary care services. We currently provides high-quality medical care out of two locations - Hayward and Fremont. We have an excellent reputation in the community and offer a supportive and Provider-friendly team environment. The ideal provider will have at least five year of experience as a certified primary care NP in a clinical setting.
Opportunity Highlights
Minimum 3 to 5 year of most recent and continuous experience as a certified F-NP is a must
Must have experience providing primary care services in an office setting such as a private clinic or a community health center

Monday-Friday schedule with no call

Preference for multilingual candidates. English/Spanish/Punjabi preferred
EPIC EMR experience a plus
Active and unrestricted CA full NP licensure including furnisihing license
Must have a PTAN and NPI #s
Compensation
Base Salary: $130,000 to $160,000 per year
Bonus: Up to $5,000 per year
Sign-on Bonus: Up to $10,000

Responsibilities
Practices under the standard procedure established for the NP role consistent with AmCare's policy and in accordance with requirements set forth by the California Board of Registered Nursing
Conducts age appropriate assessments of patients, including history taking and physical examination
Performs assessment/data collection in an on-going systematic manner, focusing on physiologic, psychosocial, cultural, spiritual and cognitive status; patient/family education, family involvement and patient advocacy
Consults with supervising physician or other clinicians as dictated by patient need and assessment findings
Participates with physicians and other clinicians to assure effective patient care
Reviews medical record for follow up or ordering of diagnostic studies
Establishes and maintains communication with other health care providers and departments to ensure operational efficiency and effectiveness and to promote consistency and continuity of care
Establishes effective working relationships with members of the health care team, patients, and families
Delegates appropriately and coordinates duties/activities of health care team members. Evaluates effectiveness of care given by team members
Demonstrates knowledge of and applies safety principles as identified by the institution. Performs efficiently in emergency situations following established protocols, remaining calm, informing appropriate persons, and documenting events
Demonstrates responsibility and accountability for own professional practice. Participates regularly in staff development activities department personnel. Demonstrates knowledge of legal issues, including patient confidentiality and risk management in all aspects of patient care and unit functioning
Participates in department Performance Improvement activities. Participates in nursing research activities as requested. Identifies and solves problems effectively
Demonstrates a service philosophy in all interactions with patients, families, and all members of the health care team. Demonstrates proficiency in the use of computers and telecommunication modalities to document, track and convey information
Perform other related duties as necessary
Education
Masters Degree in Nursing. Completion of a Masters Program in Family Practice NP required.
Additional Requirements
Must have a National Certification from one of the following organizations: American Academy of Nurse Practitioners; American Credentialing Center or other recognized NP Certification bodies.
