The Recovery Research Network Foundation (TRRNF) is seeking a dedicated and resourceful Case Manager to join their team. Here are the details of the position:
Position Overview:
Program Focus: Working within the community with individuals living with substance use or co-occurring disorders, potentially involved in the justice or child welfare systems.
Reporting Structure: The Case Manager will FMT Clinical Director/Team Lead.
Qualifications: Bachelor’s Degree in a related field or Certified Peer Recovery Specialist. Experience providing quality care to patients with mental health and co-occurring substance use disorders is required.
Responsibilities:
Needs Assessment and Case Planning: Conduct BPS and needs assessments, formulate effective treatment plans, and assist patients in reaching their personal goals.
Individual Sessions: Conduct individual sessions to support patients in achieving their objectives. Coordinate services such as healthcare, housing, and social assistance for patients
Service Coordination: Coordinate services with the multidisciplinary team including Therapists, Peer Support, Psychiatric, and Medical Services. Advocate for clients' rights and access to services, collaborate with other professionals, such as social workers and healthcare providers and mental health court to ensure comprehensive care.
Compliance: Ensure patient needs are met in accordance with center policies, federal and state regulations, and applicable standards of care.
Qualifications:
Education: Bachelor’s Degree in a related field or Certified Peer Recovery Specialist.
Experience: Minimum of 2 years of experience working in behavioral health. Experience working with diverse populations and understanding their unique needs. Knowledge of Medicare, insurance, behavioral health, child welfare, and addiction counseling. Familiarity with crisis intervention techniques. Strong communication and interpersonal skills for effective patient engagement. Ability to use motivational interviewing to inspire positive change. Previous experience in social work, childcare, mentoring, or housing placement is a plus.
Collaboration: Ability to work collaboratively in a team as well as independently.
Skills: Experience with resource coordination, job development, job readiness skills, securing entitlements, excellent verbal and written communication skills, excellent computer skills including EMR usage to maintain accurate records. Sound understanding of various insurance plans (Medicaid, Medicare, Commercial) and experience with benefit verification.
Background Checks: Ability to pass local and Level II background checks, Palm Beach Sheriff's Office background check, and follow protocols for safety and security within a detention center.
Language Skills: Proficiency in English and Spanish preferred.
Job Type and Salary:
Type: Full-time.
Salary: $45,000.00 to $50,000.00 per year.
