Large managed care company looking for Family Nurse Practitioners for In-Home Health Risk Assessments. New grads encouraged to apply!!
Work Location: Must be willing to travel up to a 100 miles radius from your home zip code, working in Lexington, KY. Reimbursed for mileage at .70 cents per mile
Pay: $100.00 – per chart submitted
Contract Dates: 2/10/2025 to 12/31/2025
Hours: 8-5, Mon - Sun (2-3 days a week)
RESPONSIBILITIES
- Perform health risk assessments which are essentially history and physicals done within the patient’s home or on occasion in nursing home or other health facility; on occasion some of these assessments are done through a telehealth visit using simultaneous video and audio communication.
- Wellness exams that include doing A1C and using portable eye camera, this will be a main focus for them to conduct the in-home testing
- The provider will educate patients and/or patient’s family on chronic medical conditions, preventative care, and medication adherence compliance
- Provider will not be required to prescribe medicine nor order any lab or diagnostic testing
- Provider will occasionally need to obtain samples for diagnostic screening tests such as finger stick for HgbA1C, LDL, and Lead levels; urine analysis may also be required based on client need.
- Provider must be able to interpret laboratory data and diagnostic testing when available to assist in diagnosis of medical conditions
- As part of the assessment, the provider must do ADL and Fall Risk evaluation, Depression and Nutritional Health Screening, BMI evaluation, and medication reconciliation
- Scheduling based on needs and NP’s availability – this is not a full time position and there are no guarantees of weekly needs. Needs fluctuate throughout the year.
EDUCATION
- Training as a Family NP is required. Can consider adult if no pediatric patients will be seen
- Candidates must be board certified by the ANCC or AANP
- Must have active/unrestricted license in state of position opening
- New graduates will be considered
- Must be able to be credentialed by Medicare, Medicaid, and other Private Insurance Companies
- For states/markets that require state specific prescriptive authority in addition to the Federal DEA registration, NPs must be able to see patients without the physical presence of a collaborative physician (where such collaboration is required). This may mean the provider must have already accumulated the required hours of working as a Nurse Practitioner for whichever timeframe is specified by the state for which the provider is licensed
QUALIFICATIONS
- Experience with electronic medical records / electronic health records is a plus
- Required is proficiency with various computer based applications such as MS Office (Logging into programs, MS Word, MS Excel, MS Outlook, general use of mouse and navigation of different programs, typing) along with utilizing various websites to perform the required duties.
- Strong background/interest in primary care and preventative medicine is a plus
- Must have use of own car with adequate automobile insurance
- Must have home internet connection
- Excellent clinical assessment and analytical skills
- Proven ability to perform comprehensive, adult/geriatric/pediatric/postpartum physical exams
- Demonstrated ability to identify, diagnose and document chronic disease in adults
- Provider must be familiar with preventative medicine in particular cancer screening recommendations
