    Full job description

    Avalon Medical Center is seeking a full time/part time/contract board certified/eligible family medicine physician to join our growing multi-specialty practice.

    Essential Duties and Responsibilities

    Providing general primary care services to patients.

    Diagnosing, treating and managing both acute and chronic conditions.

    Prescribing and refilling medications as appropriate.

    Actively communicating with patients, families, other specialists and facility personnel (specialists, social workers, etc).

    Ordering and interpreting diagnostic tests such as imaging and lab work.

    Documenting patients’ histories and treatments in EMR.

    Minimum Education:

    Degree of M.D. or D.O. - Family medicine or internal medicine

    Board Certified or Board Eligible

    Current/active New York State Medical Practitioner License

    Minimum Experience:

    Professional clinical experience in primary care practice in either family medicine or internal medicine required.

    Required Certification/Registration:

    Valid NYS Medical License

    Board Certification in Internal Medicine or Family Practice

    Current and ongoing BLS certification required and ACLS preferred

    NY State DEA Certification, valid and unrestricted required.

    Current NY State Opioid Prescriber Certificate required.

    Current Infectious disease certificate required.

    Qualifications

    Experience in patient care within a medical office setting is preferred.

    Strong communication skills with the ability to explain complex medical information clearly to patients.

    Ability to work collaboratively within a multidisciplinary team environment.
