    Pay Rate: $45,000-60,000(Exempt)(Depending on years of experience)

    The Esperanza Center provides a welcoming atmosphere, social services, health services, legal support and English as a Second Language (ESL) to immigrants in the Baltimore metropolitan area. The center's staff offers assistance with employment, English proficiency, legal services, and access to primary and urgent health care and referral services. The health services are offered in partnerships with area hospitals and medical schools.

    Catholic Charities of Baltimore Esperanza Center is currently seeking a Family Nurse Practitioner - Bilingual who will provide primary care to immigrant patients of all ages at the Esperanza Center Health Clinic. Responsibilities include diagnosis and treatment of acute and chronic illness, ordering laboratory and other diagnostic tests, patient referrals and prescribing and managing medication. The current work schedule is Monday and Tuesday 8 am-6:30pm(30 min lunch) a total of 20 hours per week (Days might be Flexible if needed).

    JOB DUTIES & RESPONSIBILITIES:

    • Required to be bilingual, with fluency in both Spanish and English, and the ability to effectively communicate medical information in both languages.

    • Collaborate with medical and program staff and participate in preventative health programs to facilitate the optimal health of clients.

    • Work closely with the Medical Director, Program Director, staff and clinic volunteers, ensuring effective consultation and collaboration to achieve program goals.

    • Participate in organizing and directing grant-funded preventive health programs to facilitate optimal health, disease detection, prevention, and management of common illnesses.

    • Assist the Medical Director and Program Manager with accurate data, cases and outcomes to support the completion of grant reports.

    • Ensure patient clinical records are accurately maintained and up to date, including medications, allergies, and problem lists.

    • Collaborate with the Esperanza Health team in the development of Clinical Manuals, Policies and

    • Assess clients' needs and proactively refer them to appropriate social services to address social determinants of health.

    • Actively contribute as a participating member of Quality Assurance and Peer Review Committees, ensuring continuous improvements and compliance.

    • Must remain flexible to the needs of clients, staff and volunteers.

    • Ability to work flexible schedule as needed, to include some evening hours.

    • Perform other duties as assigned.

    EDUCATION REQUIREMENTS :

    • Master's degree in Nursing and a Certified Registered Family Nurse Practitioner (FNP) license with the Maryland Board of Nursing, with a current active unrestricted license to practice. Minimum 2 years as a Certified Registered Nurse Practitioner with relevant clinical experience.
    • Registered with Maryland Controlled Dangerous Substance (CDS) and Drug Enforcement Administration (DEA).
    • Basic understanding of primary care clinic processes, regulations, and procedures.
    • Basic Life Support (BLS), CPR, and AED certified.
    • Experience with chronic disease management, acute care, and minor procedures.

    • Requires the ability to utilize computer systems and software necessary to perform position functions.
    • Basic Windows PC, Epic Electronic Medical Record system, web browsing (i.e. Chrome, Internet Explorer, ), and Microsoft Outlook skills required. Other Microsoft Office application knowledge, such as Word, Excel PowerPoint, Teams, and OneDrive desired.

    PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:

    • Light work that includes moving objects up to 20 pounds.
    • Moving self in different positions to accomplish tasks in various environments including tight and confined spaces.
    • Remaining in a stationary position, often standing or sitting for prolonged periods.
