Key Responsibilities:
Perform comprehensive medical assessments and ongoing evaluations for clients in rehab and sober living.
Diagnose and treat acute and chronic medical conditions, including those related to substance use.
Provide medication management, including MAT (Suboxone, Vivitrol, etc.), in compliance with state and federal regulations.
Monitor clients for withdrawal symptoms, medication side effects, and overall health status.
Collaborate with psychiatrists, therapists, case managers, and other healthcare providers to develop integrated treatment plans.
Educate clients on disease prevention, wellness strategies, and harm reduction approaches.
Provide referrals for specialized care when necessary.
Maintain accurate and up-to-date electronic health records (EHRs) in compliance with HIPAA and facility policies.
Participate in staff meetings, training sessions, and case consultations to ensure coordinated care.
Ensure compliance with Kentucky state medical regulations and licensing requirements.
Qualifications:
Active and unrestricted FNP license in Kentucky.
Board certification as a Family Nurse Practitioner (FNP-BC or FNP-C).
DEA license (or eligibility to obtain) for prescribing medications, including MAT.
Experience treating substance use disorders, co-occurring mental health conditions, and chronic diseases.
Knowledge of medication-assisted treatment (MAT) and withdrawal management.
Strong understanding of trauma-informed care and harm reduction principles.
Excellent communication, documentation, and collaboration skills.
Ability to work in a fast-paced, multidisciplinary environment.
CPR/BLS certification (or willingness to obtain).
Preferred Qualifications:
Experience working in rehab, sober living, or community health settings.
Familiarity with Kentucky Medicaid, insurance billing, and documentation requirements.
Training in motivational interviewing, SBIRT, or other evidence-based approaches for addiction recovery.
Experience with electronic health record (EHR) systems.
